Position Responsibilities:
- Initiates telephone contacts with post-acute and community resources at the request of Care Manager/Social Worker to support patient discharge arrangements. Determines availability of services specified.
- Collects, copies and transmits pertinent clerical and patient demographic information required to complete arrangements for post acute care and/or placement as directed.
- Confirms post-acute arrangements with providers, communicating date/time and method of transfer. Documents communicated information according to department policy and procedures.
- Arranges post-acute transportation, DME and community services for patient in acute care setting as directed.
- Communicates post-acute service availability, barriers to timely discharge/transfer and issues requiring Social Work/Case manager follow-up to appropriate staff members.
- Maintains regular communication with Social Worker, Case Managers and Case Management leadership.
- Maintains an adequate inventory of office supplies based on usage patterns; prepares all purchase requisitions and disbursements.
- Provides secretarial and clerical support including faxing, filling and typing.
- Updates forms and reference files to ensure that they are current all times as requested.
- Screens telephone calls and identifies urgency of issues initiates appropriate referrals and pages promptly.
- Performs related clerical and record management responsibilities as required.
- Assumes primary responsibility for the delivery and distribution of mail in the department.
- Promotes service orientation in the performance of position duties and responsibilities and in interactions with patients, hospital staff and visitors.
Qualifications:
- High School Diploma or GED Preferred.
- Must have extensive knowledge of computers, calculators and general office equipment.
- At least 2-3 years of experience in clerical related duties.

Pay: $17.00 - $18.00 per hour
