Client Assessment: Conduct thorough assessments of clients' needs and preferences to create individualized care plans. Regularly review and update care plans to ensure they align with evolving needs.
Care Coordination: Collaborate with healthcare professionals, caregivers, and community resources to ensure comprehensive and coordinated care for elderly clients. Facilitate communication between all stakeholders.
Quality Assurance: Implement and oversee quality assurance measures to uphold the highest standards of care. Conduct periodic evaluations and make recommendations for continuous improvement.
Staff Training: Provide guidance and training to caregivers and staff to ensure they are equipped to deliver exceptional eldercare services. Foster a culture of empathy, respect, and dignity in caregiving.
Qualifications:
- Bachelor's degree in gerontology, nursing, social work, or a related field.
Proven experience in program coordination within the eldercare or Home Healthcare sector.
