Hiring Range: $24.94 - $30.55 per hour

Kitsap Mental Health Services (KMHS) is actively recruiting for a passionate and dedicated Developmental Disabilities Care Coordinator to join our team. As a member of our multidisciplinary team, you will play a crucial role in planning, developing, coordinating, and providing treatment, rehabilitation, and support services to our program clients. Primary Responsibilities:
- Serve as a liaison between Developmental Disabilities Administration (DDA) and other community resources.
- Facilitate communication between KMHS Care Coordinators and DDA staff for treatment continuity.
- Provide direct services, including individual, group, and family counseling, crisis intervention, and case management.
- Demonstrate knowledge of the current Diagnostic and Statistical Manual of Mental Disorders.
- Coordinate treatment with internal and external substance use treatment providers.
- Assist clients in gaining resources and education to support their autonomy and basic needs.
- Work collaboratively with community partners and advocacy support groups.
- Provide crisis intervention counseling and coordinate with Designated Crisis Responders when appropriate.
- Document client progress according to established policies and procedures.
- Consult with families and community agencies to coordinate the treatment process.
- Establish and maintain effective relationships with KMHS and DDA staff.
Minimum Qualifications:
EDUCATION: Bachelor’s Degree in Psychology, social services, or behavioral health field.
EXPERIENCE: Minimum of 1 year of job-related experience. The capacity to work well with children and families.
Preferred Qualifications: In-depth knowledge of and experience with I/DD population, outstanding communication abilities, collaboration and administrative skills, and a high degree of professional insight and knowledge.
