- High School Diploma required. Bachelor's degree in communications, health education, liberal arts, business, public administration, or related fields highly preferred.
- Previous experience working in a community-based health center or similar setting.
- Bilingual Spanish/English required.
- 1-3 years of experience with patient navigation/Community Outreach in health care/HIV prevention education.
- 1-3 years of experience with patient engagement and working with diverse populations
- Proficiency with data tracking, reporting and computers and software.
- Strong communication skills.
- Ability to multi-task.
- Acts as a resource for patients or community members who have questions or problems relating to HIV/PrEP awareness and engagement, Testing and primary care services and any additional referrals requested.
- Telephone patient engagement by Interacting with patients and/or their caregivers as appropriate to ensure continuity of care and referral follow-up to ensure access to services.
- Interface with UHP's intake staff to accept referrals for community residents interested in HIV testing, PrEP or HIV treatment for patients that are HIV positive and out of care.
- Maintain patient confidentiality and HIPPA regulations per UHP policy.
- Maintains ongoing data collection/tracking, community activities and referrals. Collects pt. feedback to improve program services, quality improvement activities and required training.
- Work closely with various UHP departments such as Nutrition, Health Insurance Enrollment, Dental, Club TIA, Project Hope, etc to facilitate HIV prevention workshops, health education, nutrition education, health fairs or any other events.
- Work with UHP sites directors as needed.
