Job Description
Community Healthcare Network is seeking for a Full-time Population Health Outreach Specialist (Specialist) to ensure that patients receive quality medical care that aligns with our quality indicators. The representative works closely with the interdisciplinary team to serve as a telephone liaison between patients and their care team.
JOB FUNCTION:
Responsibilities include, but are not limited to:
- Contacts patients daily to facilitate patient compliance and quality indicators, such as required annual health screenings, and schedules appointments within appropriate time frames.
- Contacts patients post-hospitalization and emergency room visits to ensure appropriate transition of care from the hospital back to the community.
- Extracts data from internal and external data platforms to compile patient outreach lists, as directed.
- Maintains consistent communication with patients, clinicians, and the care team to ensure timely care of patient’s chronic illnesses.
- Answers multiple telephone calls appropriately and ensure timely follow-up on return calls.
- Receives incoming emails and faxes from laboratory, physician offices, hospitals, and diagnostic centers and assigns to appropriate parties.
- Scans incoming medical documents, review images for quality and upload to the patients’ medical record.
- Reviews individual clinical records for presence of adequate and required documentation, using chart review tools.
- Prepares weekly data collection and quality trending reports.
- Involvement in Performance Improvement activities.
- Facilitates related services for health center patients as assigned.
- Performs other related duties as assigned.
