Openhouse + On Lok Community Day Services
On Lok is a not-for-profit organization founded in the early 1970s by a group of citizens concerned about the plight of elders and the lack of long term support options in the community. In 2019, On Lok joined in partnership with Openhouse, a service provider for LGBTQ seniors, to open a visionary adult day program intentionally designed with and for LGBTQ seniors.

POSITION SUMMARY:Under the direction of the Director of Senior Programs and Operations, On Lok Day Services, the Community Outreach & Intake Nurse is responsible for the intake process and outreach efforts for Openhouse+ On Lok Community Day Services (CDS). The Community Nurse works closely with CDS program and operations team to ensure a smooth enrollment and intake process and provides clinical consultation and grant reporting as needed. In collaboration with org-wide enrollment and outreach teams, the Community Outreach & Intake Nurse will also strengthen community relationships and presence by providing visibility within the community, enhancing program awareness, and directing referrals appropriately.

DUTIES / RESPONSIBILITIES:
CLINICAL PRACTICES
•Leads outreach and enrollment process for Community Day Services according to regulatory guidelines set for forth by Department of Social Services
•Provides care bridging and clinical expertise to the program team in the facilitation of care planning, care coordination, and risk management by working with the PACE program directors and interdisciplinary teams to ensure continuity of care for PACE (Program of All-Inclusive Care for the Elderly) participants served in the ADP.
•Conducts intake, assessments, home visits to determine whether candidate meets adult day program eligibility and coordinates center visits, assessments as needed.
•Conducts medical record review.
•Provides oversight to ensure completion of required enrollment documents and assists with navigation of enrollment process with clients and circle of support, including individualized services plans.
•Demonstrates expertise in level-of-care and eligibility criteria.
•Assists with care coordination, review of annual and change of condition paperwork.
•Completes documentation for services provided promptly through electronic health record.
•Provides Clinical Consultation, other projects and duties as assigned.
COMMUNITY OUTREACH & ENROLLMENT
•Works closely with Adult Day Program & Operations team, community partners Openhouse and Shanti and referral sources to meet partnership goals.
•Strong advocate, strategic and focused on meeting census growth and organizational goals.
•Provides health education to community groups related to geriatric care, disease prevention and general health-related topics including senior housing developments, community senior centers, and community health fairs.
•Coordinates and conducts professional presentations to community groups, which include physician groups, universities, and schools and community-based organizations.
•Works closely with Outreach and Enrollments teams from both PACE and Day Services teams to actively promote On Lok Programs and facilitate referrals.
•Strengthens existing community partnerships and cultivate new ones through networking community presence and visibility
COMPLIANCE
•Upholds the standards of the California Nurse Practice Act, American Nurses Association Standards of Nursing Practice and Code of Ethics for Nurses.
•Practices nursing care in accordance with HIPAA (Health Insurance Portability & Accountability Act) standards and On Lok policies and procedures. Acts as a mandated reporter as defined by On Lok's policies and guidelines and those requirements as defined by CA Board of Registered Nurses.
•Adheres to infection control guidelines as stated by On Lok's policies, procedures, and guidelines.
•Upholds standards and compliance as set forth by the Department of Social Services that oversees regulatory compliance for Adult Day Programs.
SAFETY MONITORING, TRAINING & REPORTING
- •Actively participates in process improvement initiatives supporting On Lok's mission and goals.
•Actively participates in improving quality metrics for the assigned center and meeting organizational goals.
•Adheres to standards, policies, and procedures of On Lok.
•Adheres to safety policies and guidelines of On Lok.
•Attends state review visits and Department of Disability and Aging Services Site visits as needed.
•Prepares reports related to grants and contracts, as assigned. Includes the Department of Disability and Aging Services /Office of Community Partnerships, other government agencies and foundations.
•Tracks and prepares reports needed to monitor enrollment, Outreach & program effectiveness including but not limited to dashboards, excel sheets, Customer Relations Management (CRM) platforms, Electronic Health Record and PowerPoint presentations. - Provides education, training and mentoring to program staff.
- Acts as a resource, preceptor and mentor to new employees, students, and other team members.
- Provides center coverage as needed.
- Other related duties as assigned.
QUALIFICATIONS (knowledge, skills, abilities):
- California RN license Upon Hire
- Life SupportFIRSTAID - First Aid Upon Hire
- Life SupportBLS - Basic Life Support
- Non ClinicalDL NUMBER - Driver License, Valid and in State
- Demonstrated ability to maintain an upbeat, positive approach to coordination, reinforce positive behaviors and create a highly effective team
- Working knowledge of Medicare and Medical, Department of Social Services policies and regulations

Your final compensation offer will be determined based on factors such as skills, experience, education, and licenses/certifications. The pay range is expressed as an hourly rate regardless of FLSA status. Exempt positions are paid salaried and are not subject to overtime.
