Description
YR Media recently launched its Midwest Hub (Chicago) and is seeking a part-time Clinical Case Manager to work in our Media Education and Employment department. The Clinical Case Manager will provide Clinical Case management support and coordinate wraparound services. This includes supporting transition-aged youth (14-24 years old) by providing case management support, including intake, assessment, differentiated need based 1:1 case management meetings with individual wellness plans, crisis response as needed, and coordination of internal and external services and on-going care coordination, and referral services; develop relationships of trust with clients for the purpose of providing motivational counseling. This position will work with youth on site enrolled in our introductory media education programs as well as interns (in person or virtual) in our media and digital technology education training program. They will work with a diverse media education team to ensure the positive well being and development of all youth participants.
This position holds principal oversight of Clinical Case management and wraparound support services coordination of youth throughout their interaction with YR Media. The Clinical Case Manager will utilize a strength based approach to complete bio-psychosocial assessments of newly enrolled participants, co-develop individualized action plans and update assessments on a quarterly basis. The ideal candidate will have a love of working with young people, be familiar with Chicago schools and local Chicago youth organizations, be familiar with youth development principles and practices, and thrive in a team environment.
Requirements
Responsibilities:

  • Provide on site tiered case management for YR Media participants engaged in the Media Education and Employment Pathway;
  • Monitor and support youth participation in advancing through YR Media's media and digital technology education training program with successful outcomes;
  • Administer bio-psychosocial assessments, Clinical Case management and counseling services;
  • Establish an individualized action plan for each participant in need;
  • Facilitate weekly individual sessions with participants in need;
  • Collaborate with outside stakeholders (i.e. parents/guardians, teachers and guidance counselors, probation officers, social workers, etc.) as needed;
  • Maintain documentation requirements (individualized action plans, progress notes, incident reports, grant reporting data, etc.);
  • Support youth in managing the social services sector (i.e. accessing public benefits, housing, food access, etc.);
  • Serve as a liaison and advocate between YR Media staff and youth participants;
  • Provide information and referrals to off-site service providers that can meet the support needs of youth beyond YR Media's capacity and scope;
  • Provide crisis intervention, conflict resolution and clinical counseling to individuals;
  • Promote youth self-sufficiency and independence;
  • Comply with all mandated reporting procedures;
  • Maintain detailed, confidential records on all participants served, and collaborate with management on all data collection and reporting requirements (such as service hours completed for government grant requirements);
  • Attend meetings, trainings, workshops, as assigned, to build collaboration with outside stakeholders and for ongoing professional development;
  • Participate in YR staff meetings and lead or organize organization-wide training on best practices in supportive services for youth;
  • Other related duties as assigned.

Required Qualifications, Knowledge & Skills:
  • Bachelor's degree in social work, psychology, public health, counseling or equivalent education and experience REQUIRED. Master's degree preferred
  • Clinical license as LCSW or MFT REQUIRED (Intern-level working toward licensing hours is okay)
  • Minimum of 3-5 years direct service experience with youth
  • Proven experience in and knowledge of case management methods, principles, techniques and resources (including information/referral, intake, assessments, interviewing, client advocacy, crisis and risk intervention, resource utilization)
  • Ability to quickly adapt to new technology, automated systems and databases
  • Experienced in supporting system-engaged or previously system-engaged youth in achieving goals
  • Well organized, detail oriented and ability to handle a variety of tasks
  • Ability to work independently, take initiative, prioritize duties and work closely with several departments
  • Strong written and verbal communication skills

Compensation: This is a part-time, non-exempt position working 25 hrs/week. There may be opportunities to work more hours plus expand the role to a full time position in the future. The hourly wage rate will be commensurate with experience. The hourly pay range for this position is $30 - $32.
Supervisory:
This position reports to the Director of Media Education & Employment.
Location & Physical Requirements
Employee's principal place of work is Chicago, IL. As we launch our Midwest Hub, this position is currently a hybrid role and will transition to regular on-site presence at the Chicago office. Approximately 90% of the time spent on the job involves use of a monitor; therefore, employee must be able to sit for extended periods and to utilize standard ergonomics practices. Unless otherwise traveling, employee will primarily work in an office environment, requiring the following activity: lifting and carrying materials weighing up to 25 pounds, kneeling, bending. Must be able to use standard office equipment.
