Duties & Responsibilities:

• Delivers short-term Clinical Case management services, creating and monitoring service plans.
• Engages in regular meetings with assigned cases and collaborates with community representatives.
• Provides individual counseling to address motivational and psychosocial challenges.
• Be a lifeline through intense advocacy, crisis intervention, and problem-solving.
• Conducts outreach, and intakes, and makes referrals to empower our program participants.
• Screens for income support, advocating for benefits on behalf of our community members.
• Develops and oversees treatment plans, ensuring realistic goals are achieved.
• Extends financial assistance and management support in alignment with our organizational policies.
• Cultivates professional relationships with governmental, health, and social service agencies.
• Participates in supervision and training sessions, contributing to our collaborative team.

Qualifications:

• Master’s degree in Social Work, Counseling, Clinical Psychology, or a related field.
• Minimum 3 years of experience in Social Work, Counseling, Clinical Psychology, or a related field.
• LPC/LSW license is required.
• Valid driver's license.

Knowledge/Skills/Abilities (K/S/A)

• Excellent written and verbal communication skills.
• Strong customer service and organizational abilities.
• Knowledge of local support service providers.
• Proficiency in Microsoft Office and relevant software.
• Commitment to a strength-based, trauma-informed, and human rights-grounded approach.
