Duties & Responsibilities:
- Delivers short-term Clinical Case management services, creating and monitoring service plans.
- Engages in regular meetings with assigned cases and collaborates with community representatives.
- Provides individual counseling to address motivational and psychosocial challenges.
- Be a lifeline through intense advocacy, crisis intervention, and problem-solving.
- Conducts outreach, and intakes, and makes referrals to empower our program participants.
- Screens for income support, advocating for benefits on behalf of our community members.
- Develops and oversees treatment plans, ensuring realistic goals are achieved.
- Extends financial assistance and management support in alignment with our organizational policies.
- Cultivates professional relationships with governmental, health, and social service agencies.
- Participates in supervision and training sessions, contributing to our collaborative team.
Qualifications:
- Master’s degree in Social Work, Counseling, Clinical Psychology, or a related field.
- Minimum 3 years of experience in Social Work, Counseling, Clinical Psychology, or a related field.
- LPC/LSW license is required.
- Valid driver's license.
Knowledge/Skills/Abilities (K/S/A)
- Excellent written and verbal communication skills.
- Strong customer service and organizational abilities.
- Knowledge of local support service providers.
- Proficiency in Microsoft Office and relevant software.
- Commitment to a strength-based, trauma-informed, and human rights-grounded approach.
