You love working with some of Chicago’s most vulnerable—people who have experienced chronic homelessness and live with a disability, physical or mental illness, or struggle with substance abuse. You have clinical skills. You are prone to see strengths. You know that good record keeping helps people succeed. And you want to be part of a thoughtful team that meets participants where they are, helps them articulate meaningful goals and helps them accomplish those goals.
Position Summary
The Clinical Case Manager is responsible for providing direct clinical and case management services, including: intake and assessment with tenants, collaborative development of individual service plans with residents, crisis intervention, individual counseling and providing therapeutic group activities to help tenants meet their goals. The Clinical Case Manager provides linkages and coordinates resources/referrals. The Clinical Case manager assists tenants in goal attainment, including but not limited to: maintaining their housing, obtaining and maintaining sustainable income, and achieving health and wellness goals.
The Clinical Case Manager reports to the Director of Programs.
Responsibilities
Participant Goal-Setting
Assess participant physical, emotional, and financial needs through intake and on-going individual meetings
Utilize motivational interviewing techniques to determine participants' readiness to change and plan appropriate interventions
Assist participants with prioritizing self-identified goals and planning to achieve those goals
Evaluate progress related to goals quarterly
Provide necessary resources to participants
Provide support and guidance to participants through process of goal-achievement
Engagement Center
Participate in the effective operation of the Engagement Center including but not limited to welcoming clients, running groups, helping clients with engaging in and accessing center resources, and facilitating relationships between clients.
Use the Engagement Center as a primary location for service delivery.
Participant Advocacy
Make appropriate referrals for services based on participant goals and needs
Work collaboratively with the Director of Operations and staff regarding participant concerns via staff meetings, program log, and written updates
Provide crisis intervention when necessary
Participant Clinical Support
Help participants identify, manage and overcome mental and emotional issues; listen to participants and ask questions to help understand
Help participants create coping strategies; listen, support, and monitor the participant for progress; refer to community resources including psychiatrists, diagnostic facilities, and treatment facilities, as necessary, including for diagnosis
Plan and lead therapeutic group activities to help participants meet their goals
File/Record-keeping and Data Quality
Keep individual participant files updated and orderly, including progress notes, intake and needs assessment, service plans, releases of information and referral forms.
Maintain daily, weekly and monthly service statistics and enter this data in contact log, case management notes, and the HMIS database
Work collaboratively with the ATA, the Executive Director, and others to ensure data quality
Perform other duties as assigned.
Required trainings, skills, and competencies
Education, Experience, and Licensure Requirements
Bachelor's degree in related field; Master's degree in social work, psychology, clinical mental health counseling, marriage, and family therapy, or a related mental health field, from a school accredited by the Council for Accreditation of Counseling & Related Educational Programs
Hold or be actively working towards a license to perform in-state counseling; licensed through the National Board for Certified Counselors, the Association of Marital and Family Therapy Regulatory Boards, and/or the Association of Social Work Board

Superb written and spoken communication skills
Excellent interpersonal skills with participants, colleagues, and community resources
Comfortable with building personal relationships and dedicated to helping others
Compassionate and caring of participants
Excellent time management skills; organized and able to prioritize
Comfortable in a fast-paced environment with multiple participants
Proficient computer skills, including Google Suite (Word, PowerPoint, and Excel)
Knowledge of issues surrounding homelessness, mental illness and substance use
Ability to confidentiality handle participant information
Commitment to the mission of the ReVive Center
Working Conditions and Physical Requirements
Travel between client residence and assigned location(s)
Work is normally performed in engagement center, office or client residences
Conducts independent home visits
Sitting for extended periods is required
Intermittent physical activity including standing, walking, bending, climbing stairs, lifting (less than 10 pounds)
Functional hearing, vision, manual dexterity and hand-eye coordination or effective accommodations in place to perform job functions
