Position Summary: The Clinical Care Coordinator is an integral part of the hospice interdisciplinary team and as such is responsible to the Business Office Manager for all aspects of the completion, storing, and retrieving of all medical records in addition to accurate completion and maintenance of workflow tasks within the electronic health records system.

Job Responsibilities:
- Liaison between the Interdisciplinary Team and patients and family members to ensure collaboration and continuity of care through phone and personal interaction.
- Provides excellent customer service acting as a liaison between the hospice, caregiver, and the patient.
- Liaison with other health professionals, support services, and the community at large to promote the hospice as a high-quality service provider.
- Basic knowledge of patient data and clinical practices to aid with triaging phone calls to field staff.
- Verifies the accuracy of patient demographic information with Medicare’s common working file.
- Assigned to Medical Records completion, accuracy, and storage both written and electronic.
- Performs validation and maintenance of Payor Source Verification, Medical Records, Scheduling, and Interdisciplinary Team function.
- Maintains specific patient files for special Payor sources.
- Processes of medical orders, lab work, and prescriptions for field staff.
- Answers and transfers office phone calls to all personnel.
- Maintain inter-office supply ordering.
- Knowledge of and adherence to all policies and procedures.
- Maintains compliance by documenting accurately and timely within the organization’s electronic health record.

Education and Experience:
- High School graduate or equivalent.
- 2 years’ experience in healthcare preferred.
- Technical knowledge of electronic health records.
- Computer experience including knowledge of Microsoft Suite and/or Google Suite.
