Job Description
JOB PURPOSE:
Under the supervision of the CDC Project Manager, the Outreach Coordinator is responsible for coordinating the social network testing program, POWER Enrollment, and Retention services, and conducting community outreach following the POWER work plan.
ESSENTIAL JOB FUNCTIONS INCLUDING, BUT NOT LIMITED TO:

2. Recruitment/Outreach
• Participates in and assists in the organization of HIV testing community outreach events within the target population.
• Understands the comprehensive HIV Prevention Core Program (4 Pillars).

• Seeks opportunities in the community to expand awareness around services available and support for individuals newly diagnosed with HIV and high-risk negatives.
• Identify community recruiters and coaches in engaging social network members in HIV testing or HIV services
• Conducts ongoing community outreach
• Maps social networks of recruiters
• Engages and refers clients for HIV prevention services
• Assists in the development of outreach activities, including date, sites, time of visits, and when appropriate liaison or contact person
• Provides input in planning and evaluating activities
• Provides referrals to medical and social services for those newly diagnosed with HIV
• Provides referrals to STI and Hepatitis screening, as well as to other medical and social services
• Assist in identification and maintenance of condom distribution sites
• Collects outreach and counseling and testing data
• Maintains accurate and up-to-date activity logs and reports
• Participates in community health fairs and other community events as requested
• Participates in ongoing professional development training as identified by Project Manager
3. Testing
• Performs HIV counseling, testing, referral, and follow-up services in the community and at satellite locations.
• Receives training and maintains skills for accurate, high-quality, culturally competent HIV testing.
REQUIRED EDUCATION:
• GED or High School Diploma.
• LDH Certification as Testing Counselor within first 6 months of employment
SKILLS, LICENSURE AND KNOWLEDGE REQUIREMENTS:
· At least one (1) year of experience providing outreach to underserved community members.
· One (1) year of experience in testing, prevention, or related services to the target population
· Valid LA Driver’s License Required
· Ability to pass an auto insurance carrier review required.
