Job description

Job Purpose:

The Data Entry Coordinator performs high volume Data Entry work. Work involves entering, verifying, researching, coding, correcting, or retrieving information for program services from source documents and/or computer system with a high level of accuracy.


Essential Job Functions including, but not limited to:

  • Satisfies Core Competencies
  • Accepts responsibility for ensuring that performance meets the standards of the professional, ethical and relevant legislated requirements.
  • Adheres to the grant and programmatic guidelines and all departmental and relevant agency policies and procedures.

  • Data Collection/Entry
    • Collect data, conduct Data Entry, and prepare monthly program statistics.
    • Maintain accurate and complete client records, testing,

referrals, and outreach performance as defined by evidence-based models, local/state/federal entities, and the department.

  • Enter client data into existing data base systems.
  • Input services into appropriate software system. Verify that data is entered accurately.

  • Testing, Counseling and Referrals
  • Provide HIV/STD Counseling, Testing and Referrals to at-risk populations utilizing program testing procedures.
  • Provide specialized HIV/STD prevention education and develops individualized risk reduction plan with client.
  • Provides follow-up services for testing recipients in the form of post-test counseling, disclosure of test results and refers as appropriate.
  • Act as a resource to provide clients to other health and social supportive services as needed.
  • Assist individuals in linkage to HIV primary medical care and other support services. Verify that services were accessed post-referral.
  • Communicate effectively with clients for appointments and follow-up on referrals.
  • Maintain accurate and current process, procedural, and program, and inventory logs of specific testing activities, health materials, condoms/lube, Temperature log, Test Results, and testing controls.
  • Conduct scientific based prevention education sessions with clients receiving testing services
  • Assist with recruitment for prevention and health services

  • Other Duties
    • Receiving and scanning of program related documents for inclusion in the appropriate system
    • Participate in outreach services to target population; this includes preparation and distribution of educational, promotional and recruitment materials.
    • Maintain client confidentiality and comply with all federal and state health information privacy laws.
    • Participate in ongoing quality improvement objectives.
    • Register patients into Electronic Health Record when attending outreach and testing events.

  • Reporting and Program/Staff Development
  • Attend all mandatory meetings and trainings assigned by supervisor.
  • Collaborates with team members and leadership in writing/reviewing program and departmental policies and procedures as needed.


Required Education:

  • High school diploma or equivalent required.
  • Associate degree in related field preferred.


Skills, Licensure and Knowledge Requirements:

  • One-year experience performing Data Entry (preferred)
  • Knowledgeable in use of Excel
  • Ability to navigate between multiple systems to collect, export, and enter various forms of private and confidential data
