Title: Case Manager
Salary: $25.51 Hourly, Non-Exempt
Shift: Sunday-Wednesday 10:00am-9:00pm
Location: Stark St.
Reports To: Program Manager, Team Lead Supervisor(s)

Do Good Multnomah is a non-profit organization that partners with the community to provide supportive housing and low-barrier emergency shelter to houseless veterans in Portland, Oregon. We emphasize relationship-building, one-on-one engagement, supportive services, and direct community participation, changing the way we serve houseless veterans. THE POSITION: The Case Manager will engage with, support, and manage a caseload of vulnerable individuals with the goal of helping them attain necessary documents and eliminating barriers to housing. Duties include assisting participants with goal identification and completion, resource connection to address the eight dimensions of wellness, and community integration. This position will be a part of a team that engages participants in improving their overall health and wellbeing using a strengths-based, housing first, and harm reductive model. Case Managers working in shelter sites are additionally expected to assist with shared site operation duties such as meal distribution, cleaning tasks, and other duties as assigned.

HIPAA REQUIREMENTS: This position will have access to Protected Health Information (PHI) during the course of their work activities. Applying the minimum necessary standard of HIPAA, the designated record sets to which this position may have access include all sections of the medical record, patient demographic information in the practice management system, etc. This position is required to read the content of these records only to the extent needed to accomplish the assigned tasks.

ESSENTIAL DUTIES & RESPONSIBILITIES:

  • Process paperwork and follow steps outlined by Do Good and the Joint Office of Homeless Services (JOHS) on engaging with your assigned caseload and reporting on the health and well-being of those participants.
  • Conduct weekly case management meetings with everyone on your caseload, assisting participants with identifying their individual goals to obtain and thrive in stable housing. Advocate for particpants' needs.
  • Prepare case notes and update service tracker to clearly document all engagement with participants.
  • Assist with program intakes, progress evaluations and exits and ensure corresponding documentation is accurate and uniform.
  • Maintain accurate physical and electronic records (i.e. shelter and case notes) utilizing the Homeless Management Information System (HMIS) and Case Worthy to enter and exit participants from the databases and track services and goal progress.
  • Develop comprehensive knowledge of community and Veteran resources and maintain effective relationships with these resources to best meet participant needs.
  • Assist participants in applying for eligible benefits, including Oregon Health Plan, food stamps, job training programs, etc.
  • Maintain consistent, clear, and respectful communication with the Navigation Specialists, Team Lead Supervisors, Program Managers, and community partners.
  • Attend supervision, staff supervisions, staff meetings, and any appropriate community professional meetings.
  • Attend all required trainings as instructed by the Program Manager.
  • Participate in compliance, quality assurance, and quality improvement activities as directed.
  • With the guidance of the Program Manager, maintain and advance professional knowledge/skills to improve program effectiveness.
  • Assist in delivery of hygiene kits and essential supplies to individuals as a part of assigned caseload.
  • Assist in management of food box program.
  • Follow protocols to maintain a clean, safe environment for staff and participants.
  • Adhere to all state and federal privacy and security regulations applicable to the program, and to Do Good Multnomah’s policies and agreements regarding confidentiality, privacy, and security.
  • Perform other duties as assigned.

SKILLS & ABILITIES:

  • Knowledge of procedures, techniques, and standards to optimize individual case management practice.
  • Consistent, clear, and direct communication with participants, staff, referrals, community partners, and vendors. Ability to communicate clearly and concisely both verbally and in writing.
  • Ability to maintain accurate records and necessary paperwork.
  • Demonstrated ability to work independently, problem solve, and complete tasks in a timely manner.
  • Ability to display a non-judgmental and non-biased attitude.
  • Willingness to learn new skills and ability to continuously learn, apply training instruction, and take constructive feedback.
  • Self-motivated and detail-oriented.
  • Demonstrated ability to work with individuals with serious and persistent mental illness (SPMI) and/or substance use disorder.
  • Knowledge of and experience with supporting an escalated participant through proper trauma-informed de-escalation and engagement techniques.] Ability to maintain calm and compassion under pressure.
  • Ability to use motivational interview techniques, strength-based advocacy, and trauma-informed care.
  • Professional maintenance of appropriate boundaries with both participants and staff.
  • Ability and willingness to engage in Do Good’s equity, inclusion, and anti-racist work at the full-staff, program-specific, and individual levels.
