Case Manager (Days Or Evenings)

This position is a 1099 contract position. Hours are flexible baed on Case Manager/client need. Hours can be accrued anytime before 9pm, any day of the week. Pay is commission based; 50% of billable hours when insurance pays (range is anywhere from $60-$80/hour). EHR system and referrals are provided for the Case Manager. This is a perfect opportunity to obtain extra clinical CSWA hours. Opportunity for paid group supervision with an outside source.

Case Manager

Job Responsibilities:

• Accomplishes clients’ care by assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress; facilitating interdisciplinary approaches; monitoring staff performance.
• Admits new clients by reviewing records and applications; conducting orientations.
• Determines clients’ requirements by completing intake interviews; determining need for therapeutic medical, psycho-social, and psychiatric evaluations; reviewing therapist evaluations, treatment objectives, and plans.
• Establishes treatment programs by setting schedules and routines; coordinating services being provided; arranging resources, including transportation and escort.
• Maintains clients’ records by reviewing case notes; logging events and progress.
• Prepares clients’ discharge by reviewing and amplifying discharge plans; coordinating discharge and post-discharge requirements; orienting and training family members; providing resources.
• Improves staff competence by providing educational resources; balancing work requirements with learning opportunities; evaluating the application of learning to changes in treatment results.
• Improves treatment results by studying, evaluating, and re-designing processes; implementing changes; rewriting policies and procedures.
• Prepares various reports as requested
• Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations. Responsibility of staying up to date with licensure and CEU requirements.
• Enhances department and organization reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.

Work hours are flexible throughout the Monday-Friday work week. Case managers set their own schedule with their clients. This is a perfect position for anyone looking to pick up extra hours or looking for a change of pace from the 8-5pm traditional working model.

Education, Experience and License Requirements

• CSWA or LCSW (required)
• 2+ years in the human services field preferred
• Professional or personal experience supporting individuals with disabilities, mental illnesses, or challenging behaviors highly preferred
• Must be comfortable conducting in-home visits and assisting client's in times of crisis
• Must have a valid in-state driver’s license, provide current registration and insurance, and have a clean driving record
• Must be over 21 years old
• Must be able to provide NPI number
