Nurse Case Manager / Oncology Nurse Navigator
Overview:
We are seeking a dedicated and compassionate Case Manager to join our team.
Description
- The Oncology Nurse Navigator works with the Providers to ensure that all baseline clinical information, both diagnostic and therapeutic, is gathered together in a systematic fashion prior to the patients visit.
- Works to ensure that new patients are prepared for the testing, evaluation and potential outcome of the visits ahead of time
- Will work collaboratively with the Community Hematology/Oncology teams when a patient is referred to the department and throughout the cancer continuum.
- An oncology nurse navigator (ONN) is a professional RN with oncology-specific clinical knowledge who offers individual assistance to patients, families, and caregivers to help overcome healthcare system barriers.
- Using the nursing process, an ONN provides education and resources to facilitate informed decision making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum.
- Work with the ONS core competencies as the guide.
The ONN competencies to be performed: coordination of care; communication to patient, families, and members of the healthcare team; & patient and family education.
Patient Population: Adult Hematology- General medical oncology.
RN navigator will provide patient education and care coordination.
Community Hem/Onc is comprised of 5 clinics.
Additional Skills & Qualifications
- Current, unencumbered Oregon State Registered Nurse License (RN).
Bachelor's of Science in Nursing (BSN) - OR- Associate Degree in Nursing (required to enroll in BSN program within 3 years & complete within 5 years of hire).
- 2-3 years of Oncology experience (outpatient oncology experience preferred)
- BLS certification (MUST be through American Heart Association & must not expire during orientation).
- Must have demonstrated regular and consistent attendance at work as reported by past employer.
Hours:
SCHEDULE: Monday - Friday, 7:00 AM - 5:30 PM, 4x10s
Job Type: Full-time
Pay: $45.00 - $55.00 per hour
