Care Coordinator - Wraparound with Intensive Services (WISe)

The Program: Wraparound with Intensive Services (WISe) pairs a comprehensive team of professionals with youth and families to provide support and promote stability across life domains.
THE ROLE: The WISeCare Coordinator will maintain the integrity of the wraparound process and ensure community-based care to youth and families engaged with WISe. Care coordinators function as a core member of a collaborative care team, consisting of a mental health clinician, peer support specialist, family support specialist, and other community providers as identified by families enrolled in WISe. This role requires the ability to work effectively in a team environment, as well as within the community and with other agency staff. In this role, Care Coordinators can expect to work in homes, schools, community centers and/or the agency office, depending on the needs of families on their caseload. Communication, confidentiality and professionalism are essential parts of this position due to WISe being a team-based approach. The ideal candidate will display a compassionate nature, be knowledgeable about behavioral health and mental health care practices, and provide exceptional service. The ability to work flexible hours including evenings and participating in an on-call rotation is expected. LCSNW offers a pay differential for employees while on-call.
RESPONSIBILITIES: The primary duties and responsibilities include, but are not limited to, the following:

  • Maintain an average caseload of 10 families, with a maximum of 15 families at any given time
  • Engage and assemble a youth and family team, utilizing both formal and informal support systems
  • Assess youth/family situations and strengths to collaboratively determine the services to meet their needs through demonstrated awareness and sensitivity in care coordination services as it relates to cultural and socioeconomic characteristics of the populations served
  • Coordinate and facilitate monthly Child and Family Team (CFT) planning meetings, as well as develop an individualized cross-system care plan based on the family's strengths, needs and their preferred solutions
  • Collaborate with other members of the WISe team (clinician, peer support specialist, family support specialist) to identify how each WISe team member is addressing ongoing youth/family needs identified on cross-system care plan
  • Meet with youth/family on a weekly basis and/or as needed to assist youth/family in achieving their goals
  • Maintain timely case notes and records set by the agency's documentation standards
  • Participate in required weekly individual supervision, case consultation, and staff meetings to explore care coordination approaches for engaging with youth and families, process themes of transference/countertransference as they occur, and identify support needed for professional growth in field
  • Working alongside youth/families to create a plan for care after their transition from the WISe program
  • Travel throughout countywide service area with mileage reimbursement allocated monthly

EDUCATION AND/OR EXPERIENCE:
  • BA/BS with a major in social work, psychology or related field or Certified Peer Counselor with the State of Washington w/a minimum one-year experience
  • 1-2 years' experience in a related field/program preferred
  • Previous WISe/wraparound experience is desirable, but not required

CERTIFICATES, LICENSES, REGISTRATIONS:
  • Submit Washington State Agency Affiliated Counselor Registration Application (within 30 days of start date)
  • Cleared through the Washington State Patrol or FBI to work with vulnerable populations.
  • Reliable Transportation, Valid Driver's License, Vehicle Registration and Insurance.
  • Washington State CANS certification (within 90 days of start date)
