QUALIFICATIONS:
- Assesses and identifies the patient/family/caregiver volunteer needs.
- Assures Volunteer visits are recorded within 24 hours of the visits
- Participates as a member of the Hospice Care Team and attends IDG meetings.
- Consults and collaborates with other members of the interdisciplinary team in the development and updating of the patient care plan.
- Demonstrates continued professional growth and development through participation in education
- Assures that all documentation meets requirements for accuracy and timeliness, and reflects services provided, billing requirements, patient/caregiver needs, and confidentiality.
- Develops, implements, maintains and evaluates the hospice Volunteer program. Recommends program changes.
- Develops, implements, maintains and evaluates the Hospice Volunteer recruitment program.
- Develops, implements, maintains and evaluates the Hospice Volunteer training and activities.
- Develops, implements, maintains and evaluates a Team of volunteers who identify as a member of that group.
- Screens, selects and tracks needed processes and volunteer files, and upon completion enters the volunteers available into the Volunteer pool for assignment.
- Assigns, coordinates, schedules, and supervises volunteers in collaboration with the IDG and identified needs for volunteer services.
- Collaborates and cooperates with other departments to organize and facilitate special events/projects in areas such as of community education, health fairs, marketing, publicity, as requested.
- Facilitating effective communication between patient/family and the IDT
- Provides administrative support to ensure efficient operation of office.
- Answer phone calls, schedules meetings, and supports office visitors.
- Carries out administrative duties such as filing, data entry, copying, binding, scanning etc.
- Completes operational requirements by completing administrative projects and expediting work results.
- Ensures operation of equipment by completing preventive maintenance requirements, calling for repairs, maintaining equipment inventories, and evaluation new equipment and technology assigned.
- Provides information by answering questions and requests.
- Maintains supplies inventories by checking stock to determine inventory level, anticipating needed supplies, placing and expediting orders for supplies as assigned.
- Contributes to team effort by accomplishing related results as needed.
- Maintains office appearance and readiness at all times including office guests and staff accommodations such as beverages, snacks, toiletries, etc.
- Adheres to all Agency, state and federal policies and procedures, laws, and regulations which are relevant to job responsibilities.
- Perform other duties as assigned.

Pay: $17.00 - $24.00 per hour
