Qualifications
The Willows Case Manager/Intake Coordinator is responsible for performing a variety of duties within case management, utilization review, and community correspondence in the inpatient setting. These include, but are not limited to, daily contacts with Willows referral sources and Third-Party payers, discharge planning and aftercare coordination. The Willows Case Manager/Intake Coordinator will be a liaison between the acute and Willows adolescent female programs.
The Willows Case Manager / Intake Coordinator is responsible for
- Managing Willows applications and referral meetings
- Works with Willows patients on any case management items that may come up
- Responsible for attending off-site activities related to treatment needs
- Works with clinical team to assist with communication with DCBS and discharge planning
- Assists in discharge planning preparation as the liasion as patient is finishing programming materials
- Assists with graduations on the unit and assists with the discharge plan when a patient is graduating or discharging from program
- Demonstrates the ability to effectively communicate with community partners, clients and staff
- Demonstrates the ability to prioritize the multiple demands of the position and are able to accurately follow instructions
- Understands the documentation requirements and complete these requirements in a timely fashion
- Excellent judgment and clinical skills are essential for this position
EDUCATION and/or EXPERIENCE: Graduate from a four year college or university with a degree in a clinical field. An equivalent combination of education and experience may substitute
