Job description

Function:
This position is responsible for the case management of up to 12 to 15 youth. This person is responsible for the coordination of care for foster youth to ensure progress in all life domains by collaborating with foster parents, therapists, biological families, schools, and community partners.Organizational duties & responsibilities:
  1. The primary responsibility of all staff is to ensure the safety and well-being of all Ramey-Estep/Re-group (RE) clients.
  2. Supports the mission, vision, and values of RE. Facilitates and adheres to the agency's code of ethics, policies, and procedures.
  3. Supports all functions that attain and maintain accreditation and compliance with regulatory agencies.
  4. Supports and facilitates positive interaction with clients and staff by exhibiting both in-office and in-public when carrying out job duties: individual maturity, respect for others, a team-centered approach, maintenance of confidential information, and awareness and sensitivity to cultural and other differences in clients and staff.
  5. Exhibits effective communication skills including proper use of agency communication systems.
  6. Participates in appropriate professional development programs to attain and maintain competency.
  7. Effectively manages financial and physical resources to achieve the mission of RE.
  8. Reports incidents of abuse or potential abuse involving clients to the appropriate authorities and RE.
Essential Duties and Responsibilities:
  1. Ensures that each youth receives services appropriate to the youth's individual treatment plan.
  2. Facilitates and writes a comprehensive treatment plan within 30 days of admission and quarterly treatment plan reviews every 90 days with involvement from the youth, youth's family, foster parents, and any involved agencies for each youth.
  3. Communicates and collaborates with foster parents, service providers, schools, state workers, and biological families as appropriate.
  4. Assists the youth and their foster family with meeting the mental health needs of each youth by linking them with the needed resources in the community.
  5. Attends all treatment team meetings and is prepared to offer suggestions concerning treatment strategies.
  6. Assists in crisis intervention with youth and their families.
  7. Enhances professional growth by attending conferences and trainings.
  8. Attends in-service to provide child-care for foster youth and may be asked to provide training during in-service.
  9. Provides on-call services on a rotating schedule and will be available 24/7 for afterhours intakes, emergencies, and crises.
  10. Must complete at least one home visit, one out-of-home contact, and two state worker contacts monthly for each youth.
  11. Ensures that State, placing agency, and agency documentation and all paperwork are completed accurately and in a timely manner.
  12. Attends court, as requested, and advocates for the best interest of youth.
  13. Collaborates and coordinates with other RE programs to ensure continuity of care.
  14. Adheres to the highest ethical standards in order to fulfill the mission of the Ramey-Estep Homes.
  15. Ensures that appropriate discharge planning is in place for each youth.
  16. Perform other duties as assigned.
Working conditions/environment:
  1. 40-hour workweek, typically Monday through Friday, with a flexible schedule.
  2. Holidays, weekends, and extra hours may be required.
  3. Meals are provided for staff when on campus.
  4. Intense, unpredictable population with the possibility of verbal and physical aggression.
  5. Fast-paced environment with the need for quick decisions to deal with any crisis that may arise.
  6. Environment is both indoor and outdoor. The environment is at times loud and stressful. Location can vary between foster homes, schools, offices, and other community settings.
minimum job requirements:
Education:
Bachelor's Degree in a human services field from an accredited school is required.Experience:
Prior experience in therapeutic Foster Care is preferred.Specific Skills and
requirements:
Must be at least 21 years of age.
Strong communication and organizational skills.
Some positions may require a valid Driver's License and insurability.
Must have excellent communication and conflict resolution skills.
Must have excellent leadership skills.
Technical requirements include proficiency with Microsoft Word, Excel, PowerPoint, and any other applications used by the organization or regulatory agencies.
Ability to understand and relate to the needs of clients from diverse backgrounds.
Ability to read, write and converse in English.
Successful completion of a pre-employment drug screen.
Successful completion of a background screening.
Successful completion of a TB skin test or proof of a negative chest x-ray or other documentation.Specialized Licenses or training:
Successful completion of Excellent Foundations.
Maintain 40 hours of annual training (including 16 hours of SCM).
Maintain 16 hours of Safe Crisis Management training, meeting all physical requirements unless given an exemption by CEO or designee.
Maintain CPR/1st Aid training.Physical Requirements:
