The Support Coordinator works directly with elderly persons and adults with disabilities living in our local communities. The Support Coordinator will be responsible for conducting assessments, developing plans of care, assisting individuals with accessing community resources and monitoring delivery of approved services.
A Bachelor's degree in a Human Services field is required. Qualified candidates must demonstrate excellent writing & communications skills. Local travel is required, a valid Louisiana driver's license, auto insurance, and a reliable vehicle are a must. MDS-HC and Plan of Care Certification preferred but not required.
