The Supports Coordinator organizes services that assist people with disabilities to live independently in the community and/or achieve greater independence in their current living situation. The primary job function of this position is to coordinate all aspects of support received by individuals who are currently qualified for Community Mental Health (CMH) services. This may include services through Medicaid, CMH, and/or Department of Health and Human Services as well as working with the individual regarding medical concerns, employment, education, housing, etc. All services will be provided in locations based on consumer choice which may include consumer homes, the community, or place of employment.
Key Responsibilities
- Organize and coordinate support services to help disabled individuals live independently in the community.
- Collaborate with healthcare professionals, employers, educational institutions, and community organizations to enhance the overall well-being of the individuals served.
- Work closely with CMH services, Medicaid, and the Departments of Health and Human Services to ensure a holistic approach to support.
- Regularly assess the effectiveness of support plans and modify them as needed.
- Monitor individuals’ progress towards achieving greater independence and quality of life.
- Complete necessary documentation for reporting purposes and compliance with regulations.
Qualifications
- Bachelor’s degree in human services or a related field is required.
- At least one year experience with developmental disabilities, and mental health services provision.
- Previous case management experience is preferred but other equivalent experience will be considered.
- Travel is required with mileage reimbursed
- Ability to work independently and self-direct
- Strong organizational skills
