BASIC FUNCTION: The Structured Day Program Coordinator provides facilitation of services to person with acquired brain injury and has primary responsibility for the development and implementation of the Structured Day Treatment Program curriculum that will lead to successful acquisition of skills for greater independence and personal choice.

ESSENTIAL FUNCTIONS:
- Design and implement lesson plan to meet individual participant outcomes, group facilitation and documentation of cognitive and psychosocial development based on participant's Individualized Supports Plans (ISP).
- Direct and participate in the development of ISP and Person-Centered Supports Plans (PCSP) that meet the needs of individuals.
- Develop progressive goals and monitor progress on goals achievements; Monitor service delivery and ensure high quality services, and compliance with regulations and standard practices.
- Responsible for supervision, safety, and welfare of program participants at all times.
- Responsible for timely documentation of attendance forms, daily activities and monthly service notes, quarterly review of individual goal attainment, intake, bi-annually and at discharge.
- Collect data and document on goals and objectives and provide updates to the Service Coordinator or Funder: Communicate changes to the plan of care as appropriate
- Communicate changes in health or behavior as appropriate
- Communicate regularly with family members to exchange information in support of the individual.
- Utilize cognitive, behavioral, physical rehabilitation, job coaching techniques (especially task analysis) environmental modifications, adaptive equipment (including assistive technique), and compensatory strategies to facilitate participant's engagement in group and individual activities in order to maximize their independent performances.
- Facilitate participants' interpersonal skills to enable positive social interactions, including developing strategies for effective negotiation and healthy conflict resolution, in order to improve quality of relationship in residential, recreational, and working settings.
- Participate in the supervision and training of program staff, providing directions, delegate responsibilities/tasks to program staff.
- Complete program staff introductory and annual performance evaluations.
- Attend and participate in required training sessions, staff meetings, and special assignments as required. Assist in program evaluation and program development activities to make the program best meet the needs of the participants.

MINIMUM QUALIFICATIONS:
- Bachelor's degree in a human services field preferred, or
- High school diploma or GED and five (5) years of experience working with individuals with disabilities
- Two (2) years of administrative/supervisory experience preferred
- Experience teaching in a group or individual setting preferred
- Must have own transportation and a valid PA driver's license. Proof of valid automobile insurance and registration.
- Must obtain the Certified Brain Injury Specialist certification within first year of employment.
WORK ENVIRONMENT & PHYSICAL DEMANDS/REQUIREMENTS
- Office time can be sedentary, requiring sitting for long periods of time. Engaging with participants and facilitating community integration involves more physical active demands.
- Regularly work in community setting required regularly scheduled trips in local community.
- Sitting, standing, walking, typing, turning in chair, stooping and reaching for files.
- Ability to receive and respond to phone calls; face to face conversation.
- Ability to see and interact with text and graphical information on screens, read forms and printouts.
- Ability to use office equipment such as copiers, scanners, computers, telephones, and fax machines.
- Ability to lift 30 pounds.
