JourneyCare Home Health, formerly Addus Home Health, was founded over 40 years ago. A lot has changed since then but our core objective remains the same: providing great care for the elderly, disabled, and chronically ill, especially those who are at risk of hospitalization or institutionalization.
We started out as a home cleaning service and then began providing chore services to Chicago’s at-risk elderly population through a contract with the city. In 2016, Addus refined its strategy to focus on growth in the states in which we have a current presence while adding clinical care services to our offerings. We now provide personal care to over 42,000 patients while providing Home Health and hospice services to over 3,600 patients. We now have 210 offices in 22 states.

What You’ll Do:
- Schedules Home Care Aide as directed by client’s care plan established upon intake
- Creates work schedules by entering schedules into the system, manages changes to client schedules due to client request, illness, vacation or leaves of absence. Provides alternate coverage to ensure the client’s care plan is followed and client services are not interrupted.
- Tracks and reports daily scheduling metrics; communicates all client scheduling trends to management.
- Processes patient authorizations and communicates with central admissions, enter reauthorizations into client record and ensures chart preparation for all new clients
- Provides thorough, complete follow-through on escalated client complaints
- Maintains accurate HR system records and ensures all employee records including payroll records are current
- Ensures the appearance of the branch’s open environment is professional: neat, clean, orderly and generally free of clutter
- Maintains a high degree of confidentiality at all times due to access to sensitive information
- Maintains regular, predictable, consistent attendance and is flexible to meet the needs of the agency
Qualifications:
- Must have high school diploma or equivalent; college degree preferred.
- Preferably will have experience in scheduling.
- Must be organized and able to multitask
- Computer proficiency - MS Office
- Must be able to work well with others in a professional manner in a team oriented environment
