Scheduler Home Health - (Walnut Creek)

Health Link Home Health and Hospice, a leading Home Health agency servicing the greater San Francisco Bay Area across multiple medical specialties and patient treatments. Currently, we are seeking a self-driven Scheduling Coordinator to join our team in the Mount Diablo Valley Area. As we approach our 20-year anniversary of providing high-quality care to our patients, we are looking for someone who shares our passion for improving healthcare outcomes and patient experience.

As an essential member of our team, you will have the opportunity to work in a collaborative and supportive environment with our clinical and office staff in San Francisco and Walnut Creek. This is an IMMEDIATE job opening for our East Bay office, the ideal candidate is a dynamic, high-energy, team player ready to join our team and hit the ground running ***This is not a hybrid or remote from Home Position*** Candidates will be required to work on-site. We are looking for someone that is willing to demonstrate exemplary performance in their ability to prioritize, multitask, organize, and learn quickly. We are looking for a detail oriented individual, with an extremely pleasant demeanor who posses excellent communication skills along with being comfortable with handling a large call volume! As a Scheduling Intake Coordinator, you will perform clerical/scheduling activities that facilitate Agency operations. These activities include coordination of clinician schedules with the patients wants and needs, processing intake information, and communication with referral sources.

What You'll Do:

• Maintain schedule for services requested and provided to agency patients
• Assist with intake
• Assure that cases are filled within established time frames
• Provide appropriate notification of schedule to employee, patient, supervisor, and others as appropriate
• Respond to emergency calls and arranges schedule accordingly
• Provide direction to direct care employees and communicate with supervisors as needed to assure safe and effective coverage of patient needs.
• Report any scheduling changes and/or client concerns to supervisor; notify client of changes.
• Verify time records with schedules
• Follow up with employee on scheduling problems encountered by the on-call staff
• Perform job in compliance with Agency policies and procedures
• Attend meetings and educational programs as required
• Accept responsibility for personal and professional development
• Perform other duties assigned

Why You'll Be Successful:

• Current scheduling/intake experience, preferably in Home Health
• Detail Oriented Individual with excellent communication skills
• Extremely pleasant on the Phone & comfortable with large call volumes
• Knowledge of medical terminology
• Demonstrates strong verbal, written and interpersonal communication skills
• Strong computer skills
• Demonstrates time management, organizational skills, and ability to function with minimal direction
