Intensive Community Mental Health Recovery Nurse - Staff RN
Intensive Community Mental Health Recovery (ICMHR) services for Veterans with serious mental illness (SMI) and significant functional impairment as a part of Mental Health
Major duties include, but may not be limited to:
- The responsibility of the MHICM-RANGE nurse is to provide and direct the delivery of quality nursing care to a culturally diverse Veteran population This includes our “rural” Veterans.
- The MHICM-RANGE nurse possesses the knowledge an skills necessary to effectively apply all aspects of the nursing process within a collaborative, interdisciplinary Mental Health practice setting.
- The MHICM-RANGE nurse serves on a multidisciplinary team which provides community based case management to Veterans with serious mental illness and a history of high hospital utilization.
- The MHICM-RANGE nurse helps to coordinate plans for nursing interventions and participates in multidisciplinary treatment plans for identification of proble areas and appropriate MHICM-RANGE interventions.
- The MHICM-RANGE nurse is responsible for completion of required clinical records as well as MHICM-RANGE forms for evaluation and monitoring of service delivery and outcome.
- The MHICM-RANGE nurse serves as a primary and secondary case manager for Veterans and aseses their mental and physical status. 7. The MHICM-RANGE nurse assists Veterans with social/recreational outings in the community.
- The MHICM-RANGE nurse uses broad based clinical knowledge to effectively implement health promotion and prevention interventions, assists in managing acute and chronic illness states and assists Veterans in attainment of optimal levels of functioning through rehabilitation.
- The MHICM-Range nurse assists Veterans in the community to maintain or improve his or her health and mental capabilities by performing functions, such as maintaining medication schedues, doing health assessments, monitoring vita signs and assisting in maintaining medication boxes.
- She/he advises the Veteran on functions that are essential to personal safety, self preservation and comfort in the community.
- The MHICM-RANGE nurse assists with research studies in collaboration with other members of the MHICM-RANGE team and applies the findings to practice. She/he serves on Nursing Service Committees and task forces as assigned.
- The MHICM-RANGE nurse delivers fundamental knowledge-based case management to assigned Veterans while developing the technical competencies required for the specific practice setting.
- The MHICM-RANGE nurse possesses in-depth knowledge regarding Veteran’s illness and psychotropic medications, and provides medication education as needed.
- The MHICM-RANGE nurse has the skills to de-escalate agitated Veterans and to manage crisis situations.
- The HICM-RANGE nurse has the skills to intervene effectively and therapeutically with Veteran’s families.
- RN’s assigned to the case management program are required to have knowledge of community resources, make field visits to Veterans and must possess a valid driver’s license. The imcumbent’s driving record will be reviewed through the California Department of Motor Vehicle’s Employer Pull Notice (EPN) program annually to ensure Veteran and staff safety.
- The RN will carry out all other duties as assigned by supervisory control individuals.
- Travel may be required to meet the needs of the organization; valid driver’s license required.
Job Type: Full-time
Pay: $103,219.00 - $175,165.00 per year
