Springpoint at Home is seeking an experienced Care Management Coordinator to join our team!
Springpoint at Home offers home care and Care Management services to residents of our Life Plan Communities as well as to individuals that live in their private home. We are seeking a full-time Care Management Coordinator/Aging Life Care Advisor to join our team. At Springpoint, our CMC is a health and human services professional who acts as a guide and advocate for their clients and/or families as well as all Care Management employees. The CMC will maintain a specialized focus on issues related to aging and elder care and is responsible for daily Care Management and activity, as well as monitoring and addressing the productivity and chart audits of the Per Diem/Part Time Care Manager(s).
Responsibilities:
- Marketing and promotion of all SAH services with a focus on Care Management.
- Promotes client psychosocial wellbeing and self-determination.
- Coordinate with doctors, family members, trust officers, accountants, and other members of an older adult’s support network.
- Provide and/or arrange for transportation to/from medical appointments, social events, and other personal outings.
- Always maintain confidentiality of all client/patient related information.
- Performs diverse duties requiring knowledge of Springpoint at Home policies and procedures.
- Completes assessments, reports, and other documentation and files all as required.
- Communicates Care Management client status changes for SAH weekly Monday meeting, Care Management team meetings, or as otherwise requested.
- Provides POA/Guardian/Co-Guardian/Conservator update report at least monthly or as required and requested.
- Maintains SAH Care Management client files with EHR documentation per policy.
- Assists with coordination/notification to aides of schedule assignments when requested.
- Assist with interview processes of Care Management staff.
- Responsible for providing reports as requested by Director and/or Nursing Supervisor.
- Graduate degree in social work or gerontology.
- 2-4 years of experience in geriatric Care Management or health care required.
- Experience in general office equipment, basic business functions.
