Join a Progressive Home Health Team: RN Case Manager Needed in Meridian, Idaho
An esteemed health care client is actively searching for a dedicated RN Case Manager to join their Home Health team. This vital role encompasses managing patient care, coordinating with healthcare providers, and ensuring that patients receive the comprehensive support they need in their homes.
Your Role:
- Conduct intake assessments with patients and families to understand their needs fully.
- Work closely with an interdisciplinary care team to manage and coordinate complex patient cases.
- Develop, implement, and adjust individualized care plans based on patient assessments.
- Ensure seamless transitions for patients from hospital to home care, coordinating necessary follow-up services.
- Advocate for patients, helping them navigate healthcare systems and secure necessary resources.
We're Seeking Candidates With:
- Active and unrestricted Registered Nurse (RN) license.
- Proven experience in a case management position within a healthcare setting.
- Exceptional organizational, communication, and leadership skills.
- A valid driver's license and reliable transportation for in-home patient visits.
- At least 1 year of in-home care experience, with Home Health experience preferred.
