Optimum Health is a mobile medical clinic currently composed of 5 Physicians, 5 Advanced Practice Providers, 1 Pharmacist, and 4 Medical Assistants. We are growing rapidly and are looking to hire our first Optimum Health Case Manager who lives, or would like to live, in eastern Idaho.
You would work with our Optimum health patients mostly in patient's private homes, but also in skilled nursing and assisted living facilities.
As our Optimum Health Case Manager, you would have the following responsibilities:
- Assist our Optimum Health Providers in creating improved healthcare outcomes
- Following up and implementing prescribed orders
- Scheduling appointments for patients and coordinating transportation to specialty clinics
- Assist patients with patient medication regimen adherence
- Assist patients with shopping for groceries and hygiene products
- Assist patient with applications for needed community resources, eg: meals on wheels, government housing, etc
As an Optimum Health Case Manager, you would have the following flexibility:
