We are seeking a Clinical Case Manager to bring experience and value to our team!
Summary:
The Clinical Case Manager coordinates care by working with other team members as well as with the patients/family members to ensure needs are met. Evaluates the effectiveness of services to the patient and family on an ongoing basis.

Experience:
- Must have experience in LTC and or Skilled care setting.
Responsibilities:
- Utilize the nursing process to provide professional nursing care
- Develop plan of care
- Manage patient care
- Coordinate with other professionals as needed
