Rehabilitation Specialist/Child and Family Specialist

ChildNet Youth and Family Services, Inc. is currently recruiting for a Rehabilitation Specialist/Child and Family Specialist for our IFCCS/Wraparound Support Services program located in Long Beach, CA.

IFCCS/Wraparound Support Services is an intensive, family-centered, strength-based, community-based, individualized intensive program that promotes resiliency, well-being, and positive quality of life in a safe, stable and caring home. The Rehabilitation Specialist (RS)/Child Family Specialist (CFS) is a combination role which will be assigned high-risk youth, working in the community coordinating services needed to meet the needs of assigned intensive clients and their families. The applicable/appropriate title for note writing will be determined based on which program services, either IFCCS or WRAP, are being provided to the client. Assigned clients meet the Integrated Core Practice Model (ICPM) qualifications according to Department of Mental Health (LA and state) guidelines. The RS/CFS will be responsible for providing mental health services/EPSDT billing for clients.

The Rehabilitation Specialist/ Child and Family Specialist responsibilities include, but are not limited to, the following:

• Maintain the ICPM philosophy, that Wrap/IFCCS approach is a family-centered, strength-based, needs-driven planning and service delivery process serving high-risk youth and their families.
• Participate in the Child and Family Team. Ensure the application of family-centered practice by the whole team.
• Maintain an active caseload (assisting Facilitators) and provide supervision for assigned clients as needed.
• Participate in the 24 hour/7 day-a-week on-call system to assure that families can access support during times of need.
• Assist the Facilitator/Care Coordinator by providing direct services weekly to the client and Family as identified in the Plan of Care and MATRIX. Work directly with assigned children and their families in various environments, including in-home/out-of-home placements and in their communities as identified in the Plan of Care/MATRIX. Advocate for youth and families within their respective communities (e.g., IEP meetings, court).
• Coordinate and organize formal and informal supports to partner with families, resulting in a plan that is permanent in the community.
• Participate in required training. Seek assistance and advice when appropriate.
• Complete all required reports and documentation in accordance with county obligations and ensure integrity of all documentation.
• Assist with coordination and participation in Parent/Caregiver Support Groups.
• Abide by ChildNet policies and procedures.
• Transport clients when required.
• Perform any other duties as assigned.

The Rehabilitation Specialist (RS)/ Child Family Specialist (CFS) shall possess:

• Bachelor’s degree in a social services field, AND 6 months of experience working with high-risk youth.
• Intermediate to advanced knowledge/skills of Microsoft Office Suite, including Microsoft Outlook and other required software.
• Bilingual proficiency in speaking, reading, writing, and understanding Spanish is preferred.

All offers of employment with ChildNet Youth and Family Services, Inc. are contingent upon a post-offer, pre-hire medical clearance with an occupational physical, tuberculosis screening, and a negative drug test. A background clearance which consists of fingerprinting through the Dept. of Justice/FBI/Child Abuse Index Search is required. Underwriting requirements of this agency include keeping a valid California driver’s license, valid auto liability insurance on file, and maintaining a good driving record while employed.
