Program Overview

@HOME delivers high-quality primary care to Medicare patients in their home. The @HOME program takes a team-based approach to providing outstanding patient care to the population attributed to Oak Street Health’s Managed Service Organization (MSO). Our Care Teams - consisting of a Provider, Medical Assistant/Scribe (CIS/MA), Behavioral Health Specialist (BHS) and Mobile Operations Coordinator (MOC) - build individual relationships across a panel of about 75 patients, seen monthly. The Care Team is responsible for delivering excellent, high-touch primary care, and coordinating the care of our patients throughout the healthcare delivery system.

Role Description

Oak Street Health Regional Vice Presidents are critical leaders in our organization. This is an opportunity to lead a regional organization of significant size and to own a large P&L. Success requires strong leadership, problem-solving, flexibility, and judgment. This is a true general management role. In this role you will:

  • Lead a team of multiple At-Home markets. You will manage 4-6 direct reports, and over 50 team members. 

  • Be accountable for exceeding clinical outcome metrics, patient growth targets, and profitability goals

  • Own the development and execution of strategies to overcome obstacles and exceed goals

  • Work side by side with teams to ensure operational excellence and deliver an outstanding patient experience

  • Coach, mentor and develop team members to help them succeed

  • Partner with medical directors to build a collaborative and effective provider culture, focused on delivering outstanding patient care

  • Be a data-driven decision maker

  • Lead opening of new markets, as needed

  • Manage relationships with local health care providers, health plans, insurance agencies, etc

  • Become part of the community - build relationships with community providers and organizations

  • Support and build Oak Street Health’s mission-driven culture

  • Other duties as assigned
