Purpose of the Position: The Recovery Case Manager is a substance abuse professional that works with internal NCHC staff, community, and county-partners to connect clients with substance use resources and programs for NCHC grants and sober living programming. The Case Manager will collaborate with community partners on SUD treatment and education needs and with NCHC substance abuse/mental health residential treatment program staff to serve as a community resources liaison.

Education, Licenses, Certification, Skills and Experience Requirements:

  • Minimum Education:

Associate degree or higher in Human Services related field earned from an accredited college

  • Minimum Work Experience: One to three years' experience in case management

  • Preferred Qualifications, Skills, Education, Licenses, Certifications and Requirements:

Ability to perform an assessment, create a treatment plan, and make recommendations based on findings

Strong verbal and written communication skills

  • Driving Requirements:

Valid Wisconsin driver's license and appropriate auto insurance coverage as required by North Central Health Care.

Must have the use of a personal vehicle.

  • Preferred Qualifications, Skills, Education, Licenses, Certifications and Requirements:

Current State of Wisconsin CSAC, SAC, SAC-IT or licensed mental health professional

Bachelor’s degree or higher in Human Services related field earned from an accredited college

Any combination of education and experience that provides equivalent knowledge, skills and abilities may be considered.

Essential Job Duties and Responsibilities:

  • Demonstrates excellent organizational, interpersonal, analytical, problem solving and critical thinking skills, and the ability to carry out the goals of the grant being overseen, as evidenced by appropriate, thorough and timely completion of grant-related requirements, tasks, and documentation
  • Responsible for the recruitment of qualified clients meeting Opioid grant requirements and providing them with services and supports in their daily lives, education on treatment options and supportive services
  • Participate in community partner meetings and partnering with jail partners who have implemented Medically Assisted Treatment (MAT) programming activities and implementing and maintaining a program that offers individuals seeking/open to this substance use treatment options
  • Serve as an NCHC AODA community outreach liaison attending community events to educate and establish contacts within the community
  • Delivery of treatment services and reports to the State and or Federal Departments as required and in full agreement with leadership
  • Responsible to support individuals in obtaining appropriate housing including processing intakes and placement for sober living applicants
  • Provide education to partners, clients, and family
  • Collaborate and consult with treatment providers and community referral sources, focusing on multidisciplinary treatment approaches.
  • Assist opioid grant clients and sober living clients with case management needs on a weekly basis
  • Participate in ongoing quality assurance review activities
  • Provide clinical care as assigned, including individual, group and family counseling
  • Adheres to scheduled work hours with regular and predictable attendance
