Job Title: Program Services Coordinator
Position Type: Non-exempt, Full-time
Reporting To: Vice President of Program Services, indirectly to Chief Operating Officer.

As a Program Services Coordinator, you will play a pivotal role in coordinating Program Services, ensuring compliance, and fostering a supportive environment for our clients.
Position Summary:
The Program Services Coordinator will collaborate closely with individuals to facilitate the coordination of all Program Services, under the guidance of the assigned QIDP. They will work towards ensuring regulatory compliance while assisting individuals in achieving their desired outcomes. This role reports to the Vice President of Program Services and requires exceptional communication skills, attention to detail, and a commitment to supporting individuals with diverse needs.
Responsibilities:
Licensing:
- Conduct regular home inspections to ensure safety and quality standards are met.
- Coordinate with Program Supervisors for performance coaching and prompt resolution of maintenance needs.
- Provide requested information to regulatory staff and participate in inspections.
Stakeholder Liaison:
- Provide regular updates to guardians as requested.
- Schedule and conduct Community Support Team meetings for significant life changes.
- Maintain professional communication with stakeholders and address concerns promptly.
Service Continuity:
- Ensure individuals have uninterrupted access to financial supports.
- Assist with intake and annual risk/support assessments as needed.
Behavioral Supports:
- Conduct spot checks after behavioral incidents and ensure coaching is completed.
- Schedule behavioral consults and participate in psychiatric appointments.
- Assist in completing SST referral applications and participate in meetings.
Home and Community Based Services (HCBS):
- Ensure compliance with HCBS waiver requirements and best practices.
- Oversee personalized activity planning, communication supports, and menu planning.
- Complete HCBS data collection compliance reviews and assist with applications and registrations.
Other Responsibilities:
- Maintain an electronic telephone log of all calls made and received while on duty.
- Perform additional duties as assigned by the COO or President/CEO.
Requirements:
- At least 6 moonths experience working with people with disabilities.
- High school diploma or GED.
- Minimum age of 21.
- Valid driver's license and reliable transportation.
- Completion of DSP training requirements.
- Ability to pass background checks and S-TOFHLA reading comprehension test.
- Proficient in communication and writing skills.
- Willingness to work with individuals with diverse needs and behaviors.
- Ability to work effectively in a team environment.
- Physical capability to meet the demands of the role.
Job Type: Full-time
Pay: $22.00 per hour
Expected hours: No less than 40 per week
