Population Health Program Manager - Full-Time
Hiring Range: $67,662.40 - $84,593.60 per year

Kitsap Mental Health Services (KMHS) is seeking a Population Health Program Manager to lead transformative initiatives to enhance client care and foster recovery-oriented, person-centered services.
As the Population Health Program Manager, you will demonstrate leadership in developing, implementing, and managing Population Health and health equity initiatives. Collaborating closely with the Quality Director and other KMHS leadership, you will formulate objectives, goals, and strategies to improve care outcomes and address community needs. Primary Responsibilities:
  • Lead the development and implementation of Population Health and health equity initiatives with measurable outcomes.
  • Formulate objectives, goals, and strategies collaboratively with Quality Director and other KMHS leadership.
  • Continuously assess services and community needs, identifying problems and proposing innovative solutions based on data analysis.
  • Engage staff and leadership in continuous improvement of systems and processes.
  • Analyze and evaluate the Population Health Team to identify barriers, solutions, interpretations, and recommendations for action.
  • Develop and manage performance measurement and quality improvement activities, preparing reports on performance outcomes.
  • Manage multi-disciplinary relationships to ensure collaboration and effective operations.
  • Create systems for effective communication of data, information, and ideas.
  • Facilitate meetings at the Population Health and agency level effectively.
  • Maintain expertise in Population Health to align with programmatic changes across the agency.
  • Stay updated on behavioral health advancements to align with agency standards and philosophy of care.

Minimum Qualifications:
EDUCATION: Bachelor's Degree in in a relevant subject area such as Public Health, Psychology, Nursing, Health Administration, Social Work, Business Administration, or related field.
EXPERIENCE: Experienced (2+ years of relevant program monitoring or clinical experience)
LICENSURE / CERTIFICATION: None
Preferred Qualifications:
EDUCATION: Master's degree in a relevant subject area such as Public Health, Health Administration, Public Administration, Social Work, or Psychology.
EXPERIENCE: Expert (4+ years of job-related experience, program monitoring experience/equivalent qualification.)
Performance Requirements:
KNOWLEDGE:
  1. Maintain a working knowledge of current trends in Population Health-based interventions.
  2. Maintain a working knowledge of relevant software and graphics programs needed for program monitoring, analysis, and evaluation.

SKILLS:
  1. Demonstrates ability to provide leadership and in the mentoring of subordinates.
  2. Utilizes excellent communication and interpersonal skills to include the ability to negotiate and resolve conflicts and build teams.
  3. Demonstrates clear and effective communication.
  4. Able to manage workflow and adjust when necessary.
  5. Able to assess priorities and communicate to staff and leadership.
  6. Demonstrates organizational skills and attention to detail.

  1. Effectively manage multiple projects simultaneously and meet deadlines.
  2. Foster a dynamic, fair, collaborative, cohesive, and respectful team.
  3. Work independently and demonstrate initiative.
  4. Assist in building positive working relationships with staff of all agency departments.

Equipment Operated: Standard office equipment including computers, fax machines, copiers, printers, telephones, etc.
Work Environment: Frequent mobility and/or sitting required for extended periods.
Mental/Physical Requirements: While performing the job duties, the employee is required to walk, sit, use hands and fingers, reach with arms, talk, or listen. Peripheral vision is also required for this position.
