Job Description
This position is restricted for use for the Mental Health Service Corps (MHSC), a comprehensive workforce development program of the Mayor’s Office of Community Mental Health, that aims to build a diverse generation of Social Workers, equipped to provide behavioral health services in a variety of settings to promote and protect the well-being of all New Yorkers.
Diverse Placement Opportunities
The Primary CareMHSC Social Worker is a maximum three-year position in which clinicians will be placed in Primary Care settings (adult Primary Care, pediatric Primary Care or women's health services), working within the Collaborative Care model. The Primary CareMHSC Social Worker will work within a clinical team, providing treatment for conditions like depression and anxiety and coordinating the patient's overall treatment plan.
Placement opportunities may be available in all 5 boroughs.
Training and Supervision
MHSC Social Workers will receive and participate in training in evidence-based practices (Motivational Interviewing, Cognitive Behavioral Therapy, Dialectical Behavior Therapy, and Interpersonal Psychotherapy) in order to provide mental heath and substance use services across multiple clinical settings.
Clinicians will obtain clinical hours applicable towards LCSW licensure under structured individual and group supervision.
General tasks and responsibilities will include:
- Under close supervision, is trained in and performs the following typical tasks:
- Screens and assesses patients for mental health and substance use disorders. Facilitate patient engagement and follow-up care
- Conducts initial intake assessments and treatment plans of for patients with behavioral health needs.
- Engages clients in individual (in person or over the phone) and/or group counseling sessions using evidence-based practice interventions such as motivational interviewing, problem solving therapy, cognitive behavioral therapy.
- Supports psychotropic medication management, as prescribed by medical providers, focusing on treatment adherence monitoring, side effects and effectiveness of treatment (when applicable).
- Adheres to record-keeping in the EHR and other required systems, reports, and related correspondence and/or paperwork as per Health + Hospitals protocols.
- Makes recommendations and facilitates changes to treatment plans based upon documented progress of clients toward goals, and collaborates with the Treatment Team in
- Comprehensive Treatment Plans Interviews domestic violence victims to assess the need for concrete and/or counseling services.
- Identifies and develops working relationships with appropriate community-based, culturally and linguistically competent agencies that offer services within clients’ communities.
- Facilitates referrals for clients to concrete social services and community-based agencies, as required and ensures appropriate and timely service delivery to clients.
- Participates in weekly individual and meetings and keeps supervisor apprised of progress with clients, client needs and problems during supervisory sessions.
- Appropriately escalates emergencies to supervisory and administrative staff and reports suspected cases of abuse or maltreatment following MHSC protocols.
- Attends and participates in various clinical case conferences, in-service training sessions, staff meetings and other mandatory trainings, workshops and events, as requested by H+H.
- May assist in the provision of clinical services only under supervision of a New York State LCSW or psychiatrist.
- Works within facility offices or in the field to provide individual and group counseling, as well as, concrete social services, as required.
- Performs other related duties, as directed and with required supervision.
Minimum Qualifications
Valid license as a Licensed Master Social Worker (LMSW) issued by the New York State Education Department (NYSED).

Special Note:
- The maximum duration of assignment as a Social Worker with MHSC is three years.
- Section 424-a of the New York Social Services Law requires an authorized facility to inquire whether a candidate for employment with child-care responsibilities has been the subject of a child abuse and maltreatment report. The facility has the discretion to assign a candidate who has been the subject of child abuse and maltreatment report to a position with no child-care responsibilities.
EDUCATIONAL LEVEL:
- Master of Social Work
KNOWLEDGEABLE IN:
- Knowledge of child and/or adult development, as well as knowledge of working with individuals, groups and families. Familiarity with social services, educational, vocational, health, and families is necessary.
