The Director of PopulationHealth and Value Based Initiatives (DOPH&VBI) The Director of PopulationHealth and Value Based Initiatives is responsible for the development and implementation of Evergreen Health’s clinical Quality Improvement/Quality Assurance Program. They work with Medical Services leadership to identify strategic priorities in improving health center care quality, access, experience, and cost. They lead a multidisciplinary team to prioritize, research, design, implement, and evaluate processes to maximize population-based health outcomes and performance under value-Based Initiatives. They develop and oversee structures for continuous quality assurance assessment and monitoring of health center processes and outcomes, including those required for compliance with accreditation and regulatory requirements.
As part of the Essential Functions for this role, the Director of PopulationHealth and Value Based Initiatives:
- Responsible for the oversight of Evergreen Health’s Quality Improvement/Quality Assurance Program and its operating policies and procedures.
- Responsible for leading multidisciplinary teams to prioritize, research, design, implement, and evaluate development of strategies to achieve Health Center success in Population Health management and risk-bearing payment models.
- Responsible for the continuous monitoring and assessment of service quality, health outcomes, and utilization, with special attention to health disparities.
- Responsible for monitoring regulation and compliance with ongoing initiatives such as Patient-Centered Medical Home (PCMH), Merit-Based Incentive Payment System (MIPS) and Uniform Data System (UDS).
- Represents Evergreen Health among community partners such as managed care organizations, Safety-Net IPA (SIPA), and regional provider collaborations.
- Actively seeks opportunities to stay up-to-date and informed on current trends, innovations, and best practices in health care as they relate to their duties.
- Supports grant writing efforts that promote innovation to patient care delivery.
- They collaborate and support the team members to ensure a patient-focused approach to care.
- Demonstrates the ability to maintain a level of flexibility required to support the evolving healthcare landscape.
Qualified Candidate will have a Bachelor's Degree in a related field and five (5) years of applicable work experience -OR- a Master's Degree in Health Care Administration/Public Health or related field and three (3) years of applicable work experience. Candidate will have excellent leadership skills and ability to mentor others. They will have excellent verbal and written communication along with excellent interpersonal and customer service skills. The candidate will have the ability to prioritize tasks and to delegate them when appropriate, along with excellent time management skills. The candidate will have prior experience as a supervisor or leader. An awareness and sensitivity to the issues that underserved populations, people living with HIV, individuals with Intellectual and Developmental Disabilities, and people who use drugs face, including health disparities that affect people of color at a disproportionate rate is essential.

Required education/experience: Bachelor's Degree in a related field and five (5) years of applicable work experience -OR- a Master's Degree in Health Care Administration/Public Health or related field and three (3) years of applicable work experience. (Master's Preferred)
