According to the New York State Education Department, Office of the Professions/State Board Office for Social Work and Mental Health Practitioners, as defined in Articles 153, 154 and 163 of the Education Law, effective June 24, 2022, an individual must be:
1)Licensed as a Licensed Clinical Social Worker (LCSW) and/or Licensed Master Social Worker (LMSW) to practice social work as operated by the Department of Mental Hygiene agencies. 2)Authorized through a limited permit valid for a period of not more than twelve months who has met all requirements for licensure as a licensed master social worker or a licensed clinical social worker except those relating to the examination and provided that the individual is under the general supervision of a licensed master social worker or a licensed clinical social worker, as determined by the department. 4) Reduce black maternal mortality and make New York a model city for women's health

PROGRAM AND JOB DESCRIPTION:
New York City Department of Health and Mental Hygiene, Division of Mental Hygiene seeks (9) fulltime HEAT (Health Engagement Assessment Team) Coordinators. Coordinators must have a clinical background related to any of the following: mental health, substance use, and criminal justice involvement. This is a field-based position.
Program Background:
Health Engagement and Assessment Teams (HEAT) support New Yorkers through a case management component, outreach, and trauma response. The program was developed for the purpose of responding to referrals received of anyone who is experiencing a behavioral health challenge caused by mental health and/or substance misuse, health concerns and any stressors that may be impacting their daily functioning, with the focus to also reduce any potential touch from the criminal legal system. Teams are comprised of a behavioral health professional and a health PEER navigator (someone with lived experience of either mental health, substance use concerns, or involvement with the criminal legal system).
HEAT is operational 16 hours a day, 7 days a week throughout the 5 boroughs.

JOB DESCRIPTION:
Health Engagement and Assessment Team (HEAT) Coordinators have the primary responsibility of maintaining a caseload of individuals who are referred to the program any of the referral sources the program currently collaborates with and more (i.e., NYPD, FDNY, FBI, US Marshals, MH Court, Public, NYCWell, Parks Department, etc).
HEAT teams are operational 13 hours a day, 7 days a week, excluding federal holidays. HEAT Coordinators will be assigned one of 2 shifts (7AM-3PM or 12PM- 8PM) on one of 2 schedules providing coverage to other shifts as need. The HEAT Coordinator(s) will:
-Utilize clinical experience with homelessness, mental health, substance use and/or criminal justice system to enhance client engagement and support.
-Significant community-level work with community-based organizations, local business, community members and other assets to inform and improve interventions.
-In conjunction with a partner, HEAT Navigator, the HEAT Coordinator will assist in client needs assessment, and will provide short term case management services.
-Prioritize individuals who are identified as high utilizers of 911 behavioral health calls (3 + calls a month that result in hospital transport).
-Maintain accurate software-based documentation of service objectives and outcomes as well as other services in accordance with DOHMH and program guidelines.
-Attend program-related community, coalition, and committee meetings.
-Prepare reports as needed.
-Participate in all program-related and supervisory meetings.
-Other responsibilities as assigned by management and may include community engagement/enhanced street outreach if other teams are prioritizing other outreach assignment.

Qualification Requirements
A Master's Degree in Social Work from an accredited school of social work.
License Requirements
You will be required to have a valid Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) license issued by the New York State Department of Education within one year of the date of appointment. This license must be presented to the appointing officer at the time of appointment or, if it is obtained after appointment, at the time it is received. This license must be maintained for the duration of your employment.
If you fail to obtain your LMSW or LCSW license within one year of the date of appointment, your probationary period will be automatically extended for six months. If you fail to obtain the required license by the end of 18 months of service, you will be terminated.
Special Note
Section 424-a of the New York Social service Law requires an authorized agency to inquire whether a candidate for employment with child-care responsibilities has been the subject of a child abuse and maltreatment report. The agency has the discretion to assign a candidate who has been the subject of a child abuse and maltreatment report to a position with no child-care responsibilities.
