We serve as the population health strategist, and policy, and planning authority for the City of New York, while also having a vast impact on national and international public policy, including programs and services focused on food and nutrition, anti-tobacco support, chronic disease prevention, HIV/AIDS treatment, family and child health, environmental health, mental health, and racial and social justice work, among others.
Our Agency's five strategic priorities, building off a recently-completed strategic planning process emerging from the COVID-19 emergency, are:
1) To re-envision how the Health Department prepares for and responds to health emergencies, with a focus on building a "response-ready" organization, with faster decision-making, transparent public communications, and stronger surveillance and bridges to healthcare systems 2) Address and prevent chronic and diet-related disease, including addressing rising rates of childhood obesity and the impact of diabetes, and transforming our food systems to improve nutrition and enhance access to healthy foods
3) Address the second pandemic of mental illness including: reducing overdose deaths, strengthening our youth mental health systems, and supporting people with serious mental illness
4) Reduce black maternal mortality and make New York a model city for women's health
5) Mobilize against and combat the health impacts of climate change
PROGRAM /JOB DESCRIPTION:
New York City Department of Health and Mental Hygiene, Division of Mental Hygiene seeks five (5) fulltime HEAT (Health Engagement Assessment Team) Health Navigators, for the HEAT program. The HEAT Navigator must have first-hand lived experience related to any of the following: mental health, substance use, and criminal justice involvement. This position is a field-based position.
Program Background:
HEAT was designed to address the unique needs of individuals encountered by first responders (NYPD, FNDY, Parks, etc.) at the intersection of public safety and behavioral health. HEAT employs behavioral health professionals and persons with lived experience to engage and support individuals with mental health issues, substance use, co-occurring disorders and health issues who can benefit from time-limited interventions, support and linkage to services in the Promotion of better health and criminal justice outcomes.
The ideal candidate will be:

HEAT Health Navigator(s) have the primary responsibility of working on Health Engagement and Assessment Teams (HEAT) engaging and supporting people in the community referred by the triage desk and external partners (NYPD, FDNY, Parks Department, etc.) with issues/needs better served by a health service approach rather than public safety response.
HEAT teams are operational 13 hours a day, 7 days a week - including holidays. HEAT Navigators will be assigned one of 2 shifts (7AM-3PM or 12PM- 8PM) on one of 3 schedules providing coverage to other shifts as need. The HEAT Health Navigator(s) will:
-Utilize direct lived experience with homelessness, mental health, substance use and/or criminal justice system to enhance client engagement and support
-Engage in significant community-level work with community-based organizations, local business, community members, NYPD, and other assets to inform and improve interventions
-Work in conjunction with a partner, HEAT Coordinator, the HEAT Navigator will assist in goal identification and support the short-term case management process
-Work in tandem with the HEAT Coordinator to assist clients in reaching their health-related goals
-Prioritize individuals who are identified as high utilizers of 911 behavioral health calls (3 + calls a month that result in hospital transport)
-Prioritize referrals from the 25th and 47th precincts
-Offer direct community engagement, prioritizing neighborhoods with the highest rates of behavioral health calls to 911, drug-related hospitalizations, psychiatric hospitalizations, and rates of incarceration.
-Maintain accurate software-based documentation of service objectives and outcomes as well as other services in accordance with DOHMH and program guidelines
-Attend program-related community, coalition and committee meetings
-Prepare reports as needed
-Participate in all program-related and supervisory meetings
-Participate in other responsibilities as assigned by management
-Participate in other responsibilities to attend all program meetings and supervisory meetings.
Qualifications
1. A baccalaureate degree from an accredited college and two years of experience in community work or community centered activities in an area related to the duties described above; or
2. High school graduation or equivalent and six years of experience in community work or community centered activities in an area related to the duties as described above; or
3. Education and/or experience which is equivalent to "1" or "2" above. However, all candidates must have at least one year of experience as described in "1" above.
