Location: Must live in one of the following states to apply: New York, New Jersey, Massachusetts/New England, Pennsylvania, or Texas.

GENERAL SUMMARY

The Population Health Partnership Director will serve as a Population Health, public health, and clinical expert for NKF and will utilize that expertise to assist local NKF teams in establishing and maintaining partnerships with government, health care industry, payers and community partners. The role involves coordinating senior level interdisciplinary teams within the healthcare landscape to employ strategies that support optimized care for people at-risk for or living with kidney disease. The Population Health Partnership Director will also provide guidance to NKF staff on public health and Population Health principles and how to advance opportunities for NKF to drive impact in these areas.

PRINCIPAL DUTIES AND RESPONSIBILITIES

  • Business Development and Relationship Management:
    • Support local NKF teams in partner engagement, business development, and relationship management, with a focus on building health care and public Health Partnerships.
    • Engage senior leadership of potential partners in the discussion of kidney disease, its impact on the community, and strategies to improve kidney disease outcomes.
    • Identify opportunities for collaborations and partnerships to improve health equity and overall health outcomes for people living with kidney disease.
  • Program Implementation and New Program Strategy:
    • Collaborate with other NKF team members to develop a kidney disease data strategy for the assigned markets.
    • Develop needs assessment of assigned markets identifying the strategic needs of partners (e.g. State Departments of Public Health or Health and Human services, Integrated health systems, health plan payers, etc.) and opportunities and partners for program development.
    • Collaborating with the NKF national and local office teams, develop new program proposals, facilitate the deployment of new programs, and provide project oversight for new program implementations.
    • Liaise with local NKF teams and other departments to ensure mutual understanding of needs and timelines for specific projects.
    • Provide quality assurance review for deployed programs communicating program outcomes internally and facilitating discussion of these outcomes with our program partners.
  • Organizational Capacity Building
    • Provide training to NKF staff on public health and Population Health principles and on NKF’s opportunity to drive impact in these areas.
    • Mentor and support local teams to expand health care business development and relationship management skills involving health care and physician executives.

MINIMUM QUALIFICATIONS

  • Master’s degree in public health, health care related field, or life sciences (or commensurate work experience).
  • Demonstrated expertise in public health, Population Health and health equity collaboration and program development, experience in primary care settings is a plus. Minimum of 5 years related work experience.
  • Experience in healthcare data analysis and outcomes measurement.
  • Knowledgeable about core health equity concepts- social determinants of health, solutions to address health disparities, etc.
    • Exceptional communication skills including comfort with technical communication with healthcare/medical leadership, public health, and government leadership, and translation of technical communications to lay audiences.
    • Ability to inspire the trust and confidence of the internal and external stakeholders with whom this role interacts.

Salary Range: $80,000- 85,000 The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future.
