The Foster Care Permanency Liaison will serve as a support person for one or more Foster Care Teams. Responsibilities will focus on foster care administrative support including paperwork, file management and other assigned tasks with expectations for the effective, efficient, achievement of Teams permanency goal, within established time frames.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Assist with preparation of court hearings, ACRs, CIPPs, and other meetings.Entering CFS 502’s into SACWIS.
Submit 1425L.
Completes referral forms and other documentation necessary for the effective, efficient, achievement of the permanency goal by established timeframes.
Occasionally(emergency basis) transporting children/parents to appointments.
Track/Assist with Legal Screens.
Files court reports.
