Duties
- Interviews patients, and complete intake interview documentation to obtain the most complete case history possible.
- Effectively utilizes different outreach/support techniques to directly engage clients
- Serves as a liaison between client, staff, and community members, ensuring client needs are met
- Provide appropriate resources to assist with clients’ needs
- Follow up with clients
- Provides referral services, contacting appropriate social agencies.
- Prepares narrative, and statistical reports as necessary. May prepare forms of application for services or benefits.
- Assists in evaluation of client/patient need in order to affect the most beneficial placement
Job Requirements:
- Knowledge of case management which includes assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individual’s human service needs.
- Knowledge of mental and emotional disorders and the social, economic, and psychological implications and behavioural impact.
- Knowledge of methods and techniques of counselling and social casework.
- Knowledge of family dynamics and reactions to developmental disabilities and mental illness.
- Knowledge of availability, role and functions of community resources.
- Knowledge of interviewing techniques.
- Ability to interpret, explain and implement applicable rules and regulations.
- Ability to prepare reports.
- Ability to maintain effective working relationships with clients, families, other service agencies, public and private, as well as state and federal regulatory agencies.
- Ability to apply interviewing techniques.
- Possession of a High school diploma.
- Certified Peer Support or ability to become certified.
