Patient Care Coordinator
General:
The Patient Care Coordinator performs customer intake/registration functions for all DME businesses to include referrals from hospitals, nursing homes, and physician offices. This role is responsible for requesting appropriate medical records and verifying that all documentation is received. The Patient Care Coordinator provides timely follow up on information requested to ensure the customer will receive services as quickly as possible.

Responsibilities and Duties:
- Coordinates services for patients and referrals including, but not limited to, timely and accurate order processing, prompt delivery of equipment, warranty repairs and replacements and related follow-up to ensure service excellence.
- Accurately enters referrals and processes orders within allotted timeframe as established, and meeting productivity and quality standards as established. This involves preparing accounts in Brightree, verifying insurance, order creation, authorizations, and notes as needed in Brightree.
- Communicates with referral sources, providers, or associated staff to ensure documentation is routed appropriately for signature/completion.
- Submits prior authorizations as needed, creating sales orders, follows up with providers to get CMNs and additional documentation as needed. Follows up on documentation for compliance & working Dashboard.
- Coordinates services for patients and referrals including, but not limited to, timely and accurate order processing, prompt delivery of equipment, warranty repairs and replacements and related follow-up to ensure service excellence.
- Complies with all applicable company policies, procedures, and patient protocols. Complies with all current government regulations and professional standards respecting Patient Care.
- Delivers customer service via phone, email and face-to-face interactions. Advises patients and caregiver on insurance coverage, payment options and other related information.
- Assists with implementation of quality improvement program to meet company and accreditation standards.
- Responsible for assisting other departments as needed in absence of other employees.
- Supports branch operation functions such as inventory, records-keeping, working reports, obtaining documentation, office maintenance and cleaning, coordination of delivery techs, policy and procedural compliance with HIPAA, The Joint Commission, State Respiratory Care Board, State Pharmacy Board and all other applicable rules and regulations.
- Performs other related duties and participate in special projects as needed or assigned.
Qualifications/Experience:
- DME Billing or Intake: 1 year (Required)
- Experience in office, preferably DME or Home Health industry
- Exceptional client focus and a natural ability to build a positive rapport with patients, referral sources and staff
- Maintain a strong sense of urgency.
- Effective verbal and written communication skills
- Ability to work in a team setting as well as independently to prioritize and manage multiple duties in order to meet deadlines.
- Proficient knowledge of Microsoft Outlook and Excel
- Basic understanding of health insurance
- Courteous customer service (inward and outward)
- Analytical and problem-solving skills with a attention to detail
PHYSICAL DEMANDS:
- Occasionally lift/move up to 50 lbs.
- Ability to stoop, bend over, kneel, sit or stand for extended periods of times as needed to meet customer’s needs
- While performing the duties of this job, the employee is regularly required to sit, use hands to finger, handle or feel objects, tools, or controls; reach with hands and arms, and talk and hear. The employee is required to sit and stand.
- Specific vision abilities required by this job include distance vision, color vision, peripheral vision, depth perception and the ability to adjust focus.
