RESPONSIBILITIES:
A community health worker is a frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. A community health worker also builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy,
RESPONSIBILITIES INCLUDE:
- Serve as a liaison/link/intermediary between HIV health/social services and the community
- Facilitate access to HIV services and improve the quality and cultural competence of service delivery
- Improve health literacy through delivery of structured education sessions
- Deliver Trauma Informed Care through scripted psychoeducation classes as appropriate
- Develop, facilitate and maintain Status Neutral Community Engagement Group to ensure consumer participation in program activity
- Build individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy
QUALIFICATIONS:
- Valid NJ Driver’s License
- Excellent communication and interpersonal skills
- Can deliver effective service coordination and capacity building, displays strong outreach and navigation experience and can facilitate health education and literacy within the community
- Demonstrate an established connection with the community we serve
- Motivated and able to work well independently and will be flexible and versatile
- Previous HIV related experience preferred, compassion, empathy and tolerance are critical
