Posting Summary
Rutgers, The State University of New Jersey, is seeking a Community Outreach Worker for
NJMS- Infectious Disease Practice. The primary purpose of the Community Outreach Worker (COW) position is to support the Medical Case Management team in delivering patient-centered, comprehensive and timely high quality services that will assist in reducing avoidable health care costs and improved patient outcomes by providing support, education, outreach engagement and retention activities. This role is supports newly diagnosed or out of care HIV+ individuals to fully engage or re-engage in HIV medical care to improve their health and quality of life.
Among the key duties of this position are the following:
NJMS- Infectious Disease Practice. The primary purpose of the Community Outreach Worker (COW) position is to support the Medical Case Management team in delivering patient-centered, comprehensive and timely high quality services that will assist in reducing avoidable health care costs and improved patient outcomes by providing support, education, outreach engagement and retention activities. This role is supports newly diagnosed or out of care HIV+ individuals to fully engage or re-engage in HIV medical care to improve their health and quality of life.
Among the key duties of this position are the following:
- Responsible for identifying individuals who are HIV-positive and not receiving HIV medical care, to build trust and inform them about living with HIV.
- Provides personalized assistance to help them enter medical care, and to provide support until they are established in care.
- Provides transportation/outreach services to patients enrolled at the Infectious Disease Practice (IDP).
- Schedules appointments on behalf of patients/clients and facilitate patients appointment-keeping.
- Advocates for patient/client access of entitlement programs, treatment facilities, and other needed community services.
- Participates on teams and external committees to communicate information, resolve problems and achieve the goals of the organization.

Qualifications
Minimum Education and Experience
- Four (4) years of experience in community work, preferably involving community health related activities.
- A Bachelor’s Degree in social work or other related field may be substituted for the work experience.
- College credits may also be substituted for the work experience on a year-for-year basis.
Certifications/Licenses
- Must also have a valid New Jersey driver’s license.
Required Knowledge, Skills, and Abilities
- Must have a working knowledge of Newark and the Greater Essex County community
- Must be computer literate, specifically with Microsoft applications and database software
- Must be accustomed to diversity
Preferred Qualifications
- Bilingual in Spanish
- New Jersey Department of Health and Human Services HIV training certification

Physical Demands and Work Environment
PHYSICALDEMANDS: While performing the duties of this job the employee is required to stand, sit, walk, use hands to handle, or feel; reach with hands and arms, talk and hear. Occasionally the employee must stoop, bend and lift or move up to 25 lbs. Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus. WORK ENVIRONMENT:Moderate noise (examples: business office with computers and printers, light to moderate traffic). Exposure to blood-borne pathogens that requires use of the personal protective equipment.

Under Policy 60.1.35 Immunization Policy for Rutgers Employees and Prospective Employees, Rutgers University requires all prospective employees to provide proof that they are vaccinated against COVID-19 prior to commencement of employment, unless the University has granted the individual a medical or religious exemption.
