Position Summary:

Center For Family Services' Cure4Camden works within a trauma-informed, hope-centered framework that creates an environment that promotes the well-being of both staff and those we serve. This service, built within a continuum of CFS' Violence Prevention, Intervention, And Recovery Services, provides a range of pathways for individuals and communities to pursue, including care coordination, street outreach, violence prevention and intervention activities, and peace conversations all provided at no cost to individuals or the community.

Duties and Responsibilities:

  • Promotes and provides cultural humility client care and staff interactions that respond respectfully to the strengths and skills of diverse ethnicities and cultures, languages, socio-economic classes, disabilities, religions, genders, sexual orientation, and other diversity-related factors.
  • Provide psycho education on trauma response, adverse childhood experiences, and violence prevention/intervention.
  • Maintain fidelity of evidenced-based and evidenced-informed interventions used in Cure4Camden
  • Conduct intakes for clients and make connections to appropriate resources to meet clients' needs
  • In collaboration with the client, complete a needs assessment to develop goals and service plan. This will be reviewed with the client to assess progress.
  • Facilitates coordination of services among various agencies, advocates for and links clients to needed services.
  • Complete documentation and other reporting requirements as per the grant.
  • Participate in staff training that may include Violence Intervention, Psychological First Aid, Trauma Informed Care, the Science of Hope
  • Participate in supervision meetings and trainings and meetings.
  • Participate in multi-disciplinary team meetings, case conference meetings, and agency wide meetings.
  • Demonstrate fair and respectful treatment of all clients from various cultures, races and ethnicities; open communication; integrity and ethics; protect confidentiality and privacy.
  • Provide Care Coordination with youth at-risk for violence - Maintain a caseload of 6 to 10 youth
  • Provide intensive case management, supportive counseling, and group counseling for a minimum of 5 hours per month per youth
  • Conduct street outreach to engage youth and build relationships
  • Assist in a public health violence prevention and intervention campaign
  • Facilitate Mediations as needed
  • Facilitate prosocial activities at the Wiggins complex
  • Co-facilitate Peace Conversations with Cure4Camden's Fellowship Peace Makers
  • Facilitate Post Traumatic Stress Management in the aftermath of a critical incident
  • Facilitate "My Best Me" curriculum in schools, community centers, etc.
  • Participate in Cure4Camden experiential learning activities with participants
  • Coordinate and participate in community-based events that bring awareness to violence as public health issue and instills a message of Hope
  • Participate in Citywide coalitions, initiatives, committees
  • Working to gain trust of the community and youth so that they know why you are there
  • Continuously keep Outreach Worker Supervisor informed as to what is going on
  • Serve as linkages and support for individuals to enhance their assistance and use of opportunities and programs in the community (education, job programs, GED, drug treatment, and mentoring)
  • Advocate for youth through court testimonies, when necessary
  • Other duties as required

REQUIREMENTS:

  • High School Diploma or GED preferred.
  • Individuals with a history of gang membership or criminal background must have made a change in their life and demonstrate that they have turned away from crime
  • Must be culturally appropriate by living and/or knowledge of Camden City communities
  • Experience working with at-risk youth and gang members
  • Experience providing outreach and case management.
  • Clearly communicate ideas in both written and verbal form.
  • Ability to connect with people of varying backgrounds, appreciate and embrace diversity, and maintain confidentiality.
  • Ability to work independently combined with the ability to be a team player.
  • Excellent documentation skills, ability to set priorities, excellent follow-up, work well under pressure and deadlines.
  • Possess strong organizational skills and personal skills.
  • Demonstrate cultural humility and a strengths-based, recovery philosophy.
  • Willing to work in a flexible and hybrid environment - office, telehealth, school and community settings.
  • Work a flexible schedule including evenings and Saturdays.
  • Complete Violence Intervention and prescribed interventions written in the grant.
  • Valid driver's license, insurance, and good driving record
