Summary:
Talbot House Ministries was established as a place of hope for people who had lost hope: the hungry, people experiencing homelessness, and people with low incomes. Planting the seeds of hope is something we do with everyone who comes through our doors.
The Outreach Coordinator works under the supervision of the Case Manager Supervisor in expanding the “Seeds of Hope” Outreach project with supportive mental and medical health resources, hygiene supplies, food, clothing, and basic living needs to build rapport with those sheltered in homeless encampments.
The Outreach Coordinator will help identify highly vulnerable individuals experiencing homelessness and those in encampments, connect them to social services, administer assessment tools, and offer resources for placement into emergency and short-term residential housing if desired.

During the first two weeks of employment, candidate will need to be on-site for training, some of which will occur during the evening shift.
Essential Duties and Responsibilities:
· Conduct community outreach, focusing on the most vulnerable.
· Weekly outreach visits to encampments to foster building and supporting program participation opportunities, confers with encampment individuals about obstacles preventing participation in programs and identify means of overcoming those obstacles.
· Assist those in encampments with accessing resources and making referrals; continue to engage with unsheltered homeless individuals until shelter or permanent housing is obtained.
· Connecting individuals with appropriate permanent housing options, such as subsidized housing Section 8, VASH, permanent supportive housing, affordable and market-rate housing, and other housing opportunities.
· Researches community resources that benefit individuals experiencing homelessness.
· Provide advocacy for people experiencing homelessness when they encounter barriers.
· Maintain a calendar of outreach activities, community events, workshops, appearances, and other communication opportunities with providers and partners.
· Schedule regular outreach exhibitions in the community and provide feedback on community responsibility.
· Develop community partnerships to assist in the implementation of support groups.
· Assist in the preparation of an annual budget for community outreach activities.
· Assist in nurturing new and old relationships with collaborative partners.
· Maintain computerized client records, daily activity logs, mileage logs, and other reports as directed.
· Drive a van, complete a daily vehicle inspection report, clean the van, and maintain all safety conditions in the van.
· Attend monthly community outreach meetings.
· Other duties as assigned

1. Increase success stories and positive outreach client outcomes (i.e., finding stable housing, gaining employment, accessing ID and SSA documentation, healthcare, etc.)
2. Increase the number of new outreach encampments in Winter Haven by (3).
3. Increase the number of positive and effective community partnerships. (i.e., Polk County Sherriff, medical, mental health, insurance, and faith-based organizations.)
4. Complete accurate data reports and services tracked for grant reporting and future funding opportunities.

Education and Experience:
· Bachelor’s degree in Social Work preferred; will consider an equivalent combination of relevant education and experience.
· 1-3 years of experience in an outreach/case management position.
· Minimum of 2 years in Social Work or Human Services.
· Must possess a valid Florida driver's license.
All positions at Talbot House Ministries require mandatory ongoing training, and every employee must be available for all scheduled training. In addition, employee attendance is needed for all mandatory staff meetings.

Pay: $18.00 per hour
