Job Description
Summary of Position: The Intake Coordinator is responsible for managing referrals for Italian Home’s milieu programs including Intensive Treatment Residence, Youth Community Crisis Stabilization units, Neurodevelopment Unit (CBAT), Out of School Time, Partial Hospital and other programs as needed. These services are for children and adolescents with serious emotional disturbances or autism and their families. The Intake Coordinator will handle all aspects of managing intakes for the services, including handling all calls, gathering information and arranging intakes and other meetings as necessary. The intent of this job description is to provide a representative summary of duties and responsibilities that will be required of positions given this title. Employees may be requested to perform job-related tasks other than those specifically presented in this description.
Evenings and Weekend shifts available.
Essential Functions:
- Respond to all referral calls within required time frames.
- Obtain demographic, insurance and clinical information as required; verify insurance eligibility; complete a typed referral form and/or enter information in electronic medical record.
- Provide information to callers about our services, assist callers in finding other resources if we are unable to serve the child and family right away and maintain a waitlist as needed.
- Work closely with the billing department to resolve insurance issues and take on other administrative projects as assigned.
- Work closely with supervisors to ensure that all openings are filled right away.
- Maintain relationships with referral sources.
Knowledge/Skills/Abilities
- Ability to communicate with clinicians, referral agencies and clients effectively and with clinical sensitivity.
- Excellent computer skills required, including comfort with Word and Excel.
- Must be extremely well organized, motivated and able to work independently.
Mental Abilities: Must be receptive and responsive to supervision and professional development. Ability to be flexible and work collaboratively. Ability to work in a fast paced environment. Ability to understand differences between programs and their funding sources and utilize the information in handling calls. Must understand issues of confidentiality. Evidence of sensitivity and respect for client’s culture and socioeconomic characteristics.
Physical Abilities: Ability to sit, talk on the phone and type for extended periods. May require bending, stooping, stretching. Visual and hearing acuity to perform job related functions.
ESSENTIAL FUNCTIONS & QUALIFICATIONS
Serve as both primary therapist and case manager for the assigned caseload
Provide individual, group, and family therapy
Must be adept at understanding and providing psychiatric diagnoses consistent with DSMV
Relay all significant clinical information to collaborating treatment team members
Serve as the primary contact and liaison to all referring sources while maintaining regular contact with outside collaterals and parents/guardians
Share information with team members in a timely fashion
Attend treatment and family conferences
Participate in daily treatment planning meetings
Must complete all treatment documentation to include initial treatment plans, psychosocial assessments, CANS assessment, progress notes/discharge summaries within established timelines per agency and insurance providers
Be able to demonstrate and implement agency training and guidelines
