Intake and Service Navigation Coordinator, Family Support
Full Time, 37.5 hours per week, hourly
Location: Travel required within the Pioneer Valley

Intake and Service Navigation Coordinator Responsibilities
UCP of Western Massachusetts, a growing human services agency, works with people with disabilities and their families throughout western Massachusetts in their homes and in the community. We are looking for an Intake and Service Navigation Coordinator with strong customer service skills to act as the first point of contact with prospective individual/families. The goal of this role is to provide support, information and resources and navigation of the systems of care to families and caregivers of individuals with disabilities. This will include:
  • Meeting members or potential members in their home or community setting.
  • Promotion of UCP services, exploring goal areas, obtaining all required documentation and making appropriate referrals.
  • Supporting members at medical, legal appointments, and/or IEP and ISP meetings.
  • Partnering with educational, state and other non-profit human service organizations to enhance member support, and assisting in program and service preparation where needed.
  • Clerical responsibilities such as maintaining intake records, case notes, services and contacts, participating in quality assurance audits, compiling data and preparing reports, and maintaining current policy and procedure manuals. These duties can be done from a home office setting until UCP establishes a physical office in the Pioneer Valley.
Intake and Service Navigation Coordinator Qualifications
  • Bachelor’s Degree in related Human Services field strongly preferred.
  • Two years’ experience in human service-related employment required.
  • Experience providing program services in a human services setting preferred
  • Preference for qualified, fluent Spanish language candidates
  • Knowledge of local community resources and community-based providers and programming.
  • Ability to maintain an Adult/Pediatric CPR and First Aid certification.
  • Ability to work flexible hours as needed
  • Proficiency with Microsoft Office/Office 365-word processing, email and database skills and remote applications such as Zoom.
  • Ability to work cooperatively and effectively in a team
  • Customer service acumen
  • Ability to demonstrate a conscientious and caring attitude.
  • Ability to effectively communicate in writing and verbally based on the needs of the audience.
  • Ability to manage time, prioritize and meet deadlines with minimal direction.
  • Ability to maintain confidentiality and privacy.
  • Negative Mantoux TB test or a clear chest x-ray.
  • Valid driver’s license, safe driving record, availability of private vehicle, up-to-date car insurance
  • Cultural competency
  • Satisfactory CORI, SORI, DPPC, National Fingerprint background checks.
