MDS Coordinator (RNAC/LNAC)

Major duties and responsibilities for this position including the following:

• Development, coordination and completion of resident assessments
• Preparation of the MDS report generation and analysis
• Data transmission and development of care plans
• Assuring compliance with Federal and State regulations governing long term care
• Working closely with the inter-disciplinary team

The ideal candidate for the RNAC/LNAC position should meet the following requirements:

- Active LPN or RN license

- Minimum of 2 years of RNAC/LNAC experience in long-term care

- knowledge of MDS Assessment process, Medicare and long-term care federal and state regulations

- experience with Medicare/Medicaid reimbursement, MDS completion, clinical resource utilization and/or case management is highly desirable
