Being a Case Coordinator for our Mapleton Intensive Program program at Devereux has its Advantages!

This role provides a combination of administrative support and case management tasks for individuals and the program. You will offer support in problem resolution and team collaboration for clients in obtaining needed services while in treatment as well as after care services for education, medical and mental health services. You will work closely with primary therapist to ensure high quality service delivery. Additionally, you will assist with admission of individuals and maintenance of the medical records.

The focus of the Mapleton program is to create an environment which promotes strength, recovery and resilience by addressing a variety of complex emotional and behavioral challenges for girls ages 12-16 at the time of admission.

Everyday as a Case Coordinator/ Admin Support you will.....

  • Schedule appointments, meetings, etc., as necessary for unit/center administration.

  • Attend all admission meetings, discharge planning meetings and completes associated paperwork and forms for those meetings.

  • Complete all correspondence related to funding, discharge plans, and recent updates, problems, issues, etc.

  • Coordinate discharge process including aftercare services, family support, required documentation, submission of referral packets etc. Coordinate transportation as needed.

Devereux Pennsylvania Children’s Services (DPACS) serves children, adolescents and young adults with autism spectrum disorders, intellectual and developmental disabilities, behavioral, emotional and psychiatric challenges and disorders. Located in West Chester and Malvern Pennsylvania, services provided include psychiatric residential treatment (PRTF), acute inpatient psychiatric hospital (AIP), assessment services, community-based mental health services, community-based employment training and social skills training, and approved private school educational services to male and female children and adolescents (6-21 years of age) with diagnoses of intellectual disabilities, autism spectrum disorders, concomitant psychiatric and behavioral challenges often associated with experiencing some level of trauma in their lives.

Qualifications

EDUCATION: Bachelor’s Degree preferred.

EXPERIENCE : Some experience preferred.

PHYSICAL REQUIREMENTS: Significant amounts of walking, sitting, and standing. Some lifting and/or managing resistance required related to applying physical intervention techniques – upwards of 75-100 lbs. or more. Some repetitive wrist/hand movements, normal hearing and vision. Significant amount of use of computers and office equipment. Occasional driving of Devereux vehicles for business purposes. Ability to meet 100% mastery of Safe and Positive Approaches (S&PA) techniques.
